Background: Acinetobacter baumannii is a critical nosocomial pathogen with escalating antimicrobial resistance, particularly to carbapenems. Its ability to survive in hospital environments and acquire resistance genes poses a significant challenge in clinical settings. Carbapenem-resistant A. baumannii (CRAB) has been prioritized by the WHO for urgent research because of its association with high morbidity and mortality. Objective: This study aimed to evaluate antibiotic susceptibility profiles and characterize carbapenem resistance genes (blaOXA-23, blaOXA-48, blaOXA-51, blaNDM, blaVIM) in A. baumannii isolates from Iraqi hospitals. Materials and Methods: A cross-sectional study was conducted in Babylon and Al-Najaf hospitals (August and November 2024). Of the 125 clinical specimens, 50 A. baumannii isolates were identified using Vitek-2 and biochemical tests. Antibiotic susceptibility was assessed using the Kirby-Bauer disc diffusion method (CLSI 2024 guidelines). PCR was performed to detect carbapenemase genes. Results: High rates of resistance were observed: 100% for ampicillin, 90% for imipenem, 84% for meropenem, and 93% for blaNDM. The following resistance genes were prevalent: blaOXA-51 (86%), blaNDM (93%), blaOXA-48 (81.3%), and blaOXA-23 (76.7%). All the isolates tested negative for blaVIM. Multidrug resistance (MDR) and extensive drug resistance (XDR) were detected in 38% and 62% of the isolates, respectively. Conclusion: The high prevalence of carbapenem resistance genes, particularly blaNDM and blaOXA-48, highlights the genetic complexity that drives CRAB in Iraqi healthcare settings. These findings highlight the urgent need for robust infection control measures and antibiotic stewardship to curb the spread of XDR A. baumannii.

Carbapenemase producing Acinetobacter baumannii is frequently associated with nosocomial infections. Increasing resistance to carbapenems, may significantly reduce the choice of effective antibiotics. This study was conducted to determine the occurrence of carbapenemase producing A. baumannii isolates obtained from Najaf hospitals. Isolates were identified according to API 20NE system and more confirmed using 16Sr RNA gene. Carbapenem susceptibility was assayed by microbroth dilution and other antibiotics using disks diffusion test. Phenotypic detection of carbapenemase was performed using the imipenem-EDTA disk and modified Hodg tests. Then isolates were subjected to monoplex PCR targeting blaOXA-23, blaIMP and blaVIM genes. Twelve (1.5.%) A. baumannii isolates were recovered from clinical infections. Five (41.6%) of isolates were found to be imipenem and meropenem resistant (MIC ranged 128-256 μg mLG1 ), of which, 4 (66.6%) gave positive result with the imipenem-EDTA disk and modified Hodg test. PCR experiments showed only two (33.3%) isolates were harbored blaOXA-23 gene. The present findings suggest that emergence of OXA-23 carbapenemase-producing A. baumannii clinical isolates in Najaf hospitals

Background: Acinetobacter baumannii is a highly problematic bacterium due to its multidrugresistant nature and ability to rapidly develop resistance. Carbapenem-resistant A. baumannii (CRAB) is particularly concerning, recognized as a top priority pathogen by the World Health Organization (WHO). In this study, we aimed to assess the susceptibility of A. baumannii to antibiotics and characterize the presence of specific carbapenem resistance genes using PCR analysis. The study was conducted as a cross-sectional investigation at Al-Sader Medical City and Baghdad Teaching Hospitals between October 2022 and February 2023. A total of 59 A.baumannii isolates were collected from patients. To ensure accuracy in diagnosis, the isolates underwent thorough processing using morphological techniques, biochemical tests, and Vitek2 systems. The susceptibility of the isolates to 24 antibiotics was evaluated using the Kirby-Bauer method. Additionally, DNA extraction and PCR analysis were performed to detect the presence of carbapenem resistance genes. The study included analysis of 59 specimens collected from patients, including sputum, wound swabs, blood, and inguinal swabs. The majority of the isolates were obtained from in-patients, demonstrating a significant difference compared to outpatients. Among the studied isolates, the most prevalent carbapenem resistance gene detected was blaOXA51, present in 97% of the isolates. This highlights the substantial presence of blaOXA-mediated carbapenem resistance among A. baumannii strains in our study population. Furthermore, our findings revealed alarmingly high resistance rates against the majority of antibiotics commonly used to treat A. baumannii infections. Approximately 95% of the isolates were classified as extensively drug-resistant, indicating resistance to multiple classes of antibiotics. This poses significant challenges in terms of effective treatment options and emphasizes the urgent need for alternative strategies to manage A. baumannii infections
