مختصر البحث

The present study aimed to investigate some microbial infections and immunological parameters associated with Covid-19 patients admitted to the intensive care unit (ICU) of Al-Amal Specialized Hospital in AL-Najaf Governorate during February and March 2021. The study included 50 patients who were assigned to two groups: 20 patients aged ≤70 years and 30 patients aged ≥70 years. The method of microbial culture was adopted to isolate bacteria and yeasts by collecting sputum specimens and oral swabs from patients and cultivating them on diagnostic media and then confirming the diagnosis by Vitek. Moreover, serum samples were collected from patients’ blood to diagnose fungal infections. Thereafter, some immunological criteria were assessed, including Covid-19 diagnosis by measuring Immunoglobulin M (IgM) and IgG, as well as examining the concentration of cytokines (Interleukin 6 (IL-6) and IF ) using the enzyme-linked immunosorbent assay (ELISA) method. The results demonstrated that bacterial species Streptococcus pneumonia (n=5; 25%), Haemophilus Influenzae (n=7; 35%), and Moraxella catarrhalis (n=3; 15%) were isolated from the first group of patients (≤70 years). The recorded data pointed out that Streptococcus pneumonia (n=10; 33.3%), Streptococcus pyogenes (n=5; 16.6%), Streptococcus viridans (n=1; 3.3%), Haemophilus Influenzae (n=6; 20%), Mycobacterium tuberculosis (n=2; 6.6%), and Pseudomonas aeruginosa (n=2; 6.6%) were the isolated and identified microorganisms in the second age group (≥ 70 years). The results revealed that the isolated yeast from the first age group was Candida albicans (n=5; 25%) and Candida glabrata (n=3; 10%), while in the second age group, 1 (3.3%) Candida albicans was isolated. The results of this study proved that 30% and 10% of patients in the first and second age groups had invasive pulmonary aspergillosis co-infection by detecting Galactomannan (GM) in the blood serum (1.05±0.59, 1.25±0.38), respectively. The results indicated that IgM and IgG levels in the serum of patients in the first age group were 11.42±6.82 and 0.47±6.82, respectively. Moreover, the levels of IgM and IgG in the second age group were 14.84±9.21 and 0.12±0.11, respectively. Furthermore, IFɤ and IL6 levels were 98.37±65.70, and 146.12±46.35 in the first group, while IFɤ and IL6 were obtained at 110.69±47.60 and 133.28±116.94 in the second group, respectively. Elderly patients with severe COVID-19 are more frequently admitted to ICUs since the proportion of severe cases and comorbidities caused by a weakened immune system is higher among this age group. Secondary bacterial infections can also occur, especially Gram-negative bacteria which are among the most significant public health problems worldwide. Moroever, aspergillosis may infect patients hospitalized with COVID-19 and lead to death.

LPS is a potential Proteus virulence factor that plays a key role in pathogenesis as well as in stimulating innate immune response. The structure of lipopolysaccharide differs from one bacterium to another, dependent on enzymes and gene products that can modify the basic structure of lipopolysaccharide in some bacteria, especially pathogens. So this study aimed to detection the present waa genes (waaA, waaC, wamB, wabG) that involve in biosynthesis of core LPS. 50 isolates of P. mirabilis were isolated from200 urine specimens taken from recurrent –urinary tract infections (UTI) patients attended to AL-Sadar Hospital. Specimens were cultured on specific media, and then bacterial isolates were identified depending on morphological, biochemical and VITK-2. The results showed that the. P. mirabilis comprise 11 (22%), 30(30%) and 9 (18 %) from recurrent UTI, kidney stone and Catheters samples respectively. The isolate that appeared multidrug resistance to all antibiotics used in this study were chosen to extracted lipopolysaccharide. Then this specimen was used for studies the genes responsible for synthesis of LPS,this specimen give positive result for waaA,waaC, wamB and wabG

Proteus mirabilis is a common cause of recurrent urinary tract infections in individuals with functional or structural abnormalities. It also forms bladder and kidney stones. Lipopolysaccharide (LPS) is a potential Proteus virulence factor that plays a key role in pathogenesis, as well as in stimulating innate immune response. Therefore, this study aimed to extract LPS from a highly resistant isolate and incorporate it in a delivery system (liposome) to stimulate an immune response against virulent pathogens. In the work, 50 isolates of P. mirabilis were taken from 200 urine specimens obtained from recurrenturinary tract infections (UTI) of patients of AL-Sadar Hospital. Specimens were cultured on specific media, and then bacterial isolates were identified via morphological, biochemical and Vitek-2 systems. The results showed that P. mirabilis was expressed in 11 (22%), 30 (60%) and 9 (18%) recurrent UTI, kidney stone and catheter samples, respectively. All isolates were assessed through antibiogram testing, with the results revealing that most isolates were multidrug resistant to more than 3 classes of antibiotics. Herein, P. mirabilis NO 50 revealed particularly high resistance, so it was chosen for LPS extraction. Lethal dose 50 (LD50) observations indicated that a live suspension of P. mirabilis was at 4.5×107 CFU/ml, while LPS was at 270 μg/ml. LPS was used as an immunogenic to stimulate the immune system through injecting Rats intraperitoneally (I.P.) with 1 ml of LD50%. Subsequently, the efficiency of immunogenes in stimulating the immune response was evaluated by determining the Toll-like receptor and CD14 levels. The results indicate that LPS incorporated in the Liposome released moderate levels of Toll-like receptors-4 (TLR4) that enabled the immune system to clear pathogens. The LPS+ complete Freund’s adjuvant (CFA) and LPS vaccinated groups recorded hyper production for TLR4 (52.2 and 40.9 pg/ml, respectively), this was followed by liposome (LIP) and bacterial suspension (11 and 20.5 pg/ml, respectively) in ranking effectiveness. This study reveals a mean of CD14 that was higher in both LPS and LPS+CFA and moderate in LPS+LIP, in comparison with control and liposome groups. In conclusion, LPS-Liposomes are a promising nanomedicine for modulating the hyper response of LPS. This may lead to tissue inflammation but appeared beneficial in stimulating the immune response at moderate levels so as to eradicate infection without tissue damage

ims: Ultrasound is the initial imaging modality for the detection of salivary gland tumors. There is an argument for evaluating multiple characteristics for predicting tumors using this imaging modality because no single sonographic feature is adequate for differentiating between benign and malignant tumors. This study aimed to examine clinical and sonographic characteristics of major salivary gland masses using B-mode ultrasound and color Doppler and check the sensitivity and specificity for each parameter. Instruments & Methods: Sixty-two patients with parotid and submandibular salivary gland tumors underwent preoperative ultrasonic and postoperative histopathological evaluation. Different characteristics were examined, such as tumor shape, size, echotexture, internal calcification, cystic changes, clarity of the boundaries of the Lesion, lymph node enlargement, as well as the distribution of blood supply in the lesion by color Doppler. Findings: Histopathological examination revealed that 18 of 62 tumors were malignant and the rest 44 tumors were benign, and 68.2% of benign cases were pleomorphic adenomas. Mucoepidermoid carcinoma accounted for 61.1% cases. There was a significant difference between the malignant and benign tumors in calcification in the lesion, echogenicity, posterior echo enhancement, vascularity, and significant result in the shape of the lesion and homogeneity status (p<0.01).
