The present paper, aims to assess and compare the therapeutic efficacies of silodosin, Tadalafil, and their combination for the negotiation of large semi-arid ureteroscopic through ureteric orifice during ureteral stone management. During endoscopy, the patients in groups A, B, and C exhibited wider ureteric orifice than the patients of group D. About 70.6%, 60.6%, 62.9% and 32.4% of patients belonging to groups A, B, C, and D, respectively, exhibited easy negotiation. Significantly lower proportions of patients in groups A, B, and C (35.3%, 39.4%, and 28.6%, respectively) exhibited the need for ureteral dilation compared to those in group D (61.8%). In addition, the duration of surgery for groups A, B, and C was less than that for group D. Preoperative silodosin and Tadalafil administration in patients undergoing ureteroscopic for ureteral stones improves the access of large size duretero scope and reduced the need for ureteral dilatation with less surgical duration and minimal adverse events.

ABSTRACT— Shock wave lithotripsy (SWL) is an efficient, simple, minimally invasive therapeutic modality used to treat urinary tract stone disease. However, the pain during SWL remains a limiting factor for this procedure. Here, we compared the efficiencies and adverse effects of ketorolac, paracetamol, and Nefopam as potential analgesics during SWL. Methodology: One hundred and fifty-eight patients were recruited and divided into three groups: Group I patients received 1 g paracetamol; Group II patients received 20 mg Nefopam; and Group III patients received 30 mg ketorolac. All the analgesics were administered intravenously 30 min prior to the procedure. A visual analog scale (VAS) was employed for pain assessment. As supplemental analgesia, 1 mg/kg body weight pethidine was used. The degree of satisfaction and possible side effects of all patients were recorded before discharge. Results: Among the 158 patients, 73 (46.2%) and 85 (53.7%) patients were male and female, respectively. The mean age of the patients was 44.15 ± 11.4 years. There were no major complications during the procedure. With time, all three groups showed marked decline in VAS scores, compared to the baseline values. Four, seven, and five patients from Groups I, II, and III, respectively, complained of nausea. Compared to Groups II and III, higher proportion of patients in Group I required supplementary analgesia; however, the difference was not significant. Conclusion: The analgesic efficiencies of paracetamol and ketorolac were comparable. Furthermore, Nefopam was found to be the safest and most effective analgesic during SWL.

Introduction Cancer is one of the major causes of death worldwide. Health systems whether in developed or in developing countries like Iraq are burdened with different programs to control cancer. Our study is intended to provide information about cancer in the region of Middle Euphrates Area (MEA) of Iraq, which is one of the major areas in Iraq that exposed to the depleted Uranium (DU) at different time periods. Therefore, we are aiming to explore more information about the behavior of cancers in this region of Iraq (pattern and distribution). Aim: our study aims to describe the landscape of cancer with wide focus on the clinicopathological behavior of different types of cancers in MEA of Iraq to determine whether any differences have cropped up over time in Iraqi patients’ presentations. Patients and methods This study is a retrospective descriptive study design. Data were collected from a single tertiary cancer care oncology centre for three consecutive years from 2016 up to 2018. This Database covers nearly the entire Middle Euphrates area of Iraq. All statistical tests performed at a 95% level of significance with a two-sided p-value of 0.05 indicating statistical significance. Results and conclusion According to this study, the three most common cancers among the entire population were breast, lung, and brain cancers. Females constituted 57.0% of the entire study. Most cancers including breast cancer presented with aggressive clinicopathological behavior. Middle age groups of both sexes are more at risk of developing different cancers. Such findings are important and pave the way for future scientific cancer control programs in Iraq especially for breast cancer. The cancer appears to be flourishing in Iraq , which could be due to multiple factors. Finding a new strategy to predict the treatment response, recurrence or aggressiveness of cancers in Iraq is crucial.

-thalassemia major is a chronic, inherited hematological disease that leads to chronic anemia in the affected children. One of the options of treatment in such patients was splenectomy; however, it is not without risk of many complications; one of them is the thrombotic events. A prospective study of 55 patients with -thalassemia inscribes in this study. 14 patients (25.5%) had a normal thrombocyte count and 41 patients (74.5%) have an abnormally high thrombocytes count which was signi
