Background & objective: Diabetes mellitus (DM) is a growing epidemic with global proportions. It is estimated that in 2019, 463 million adults aged 20-79 years were living with DM. The latest evidence shows that DM continues to be a significant global health challenge and is likely to continue to grow substantially in the next decades, we attempted to shed light on the Compare between patients with diabetes mellitus type 2 in different period of the disease with respect to glucose level, HbA1c and complications correlated with diabetes mellitus and the treatment. Methodology: The study included collecting information on 80 samples from people with type 2 diabetes, which were divided into four groups, group A: 20 samples from patients with diabetes whose duration of infection was less than 5 years, group B: 20 samples from patients with diabetes who had the disease for less than 10 years and more than 5 years, group C: 20 samples from patients with diabetes who had the disease for less than 15 years and more than 10 years, group D: 20 samples from diabetic patients who had the disease for more than 15 years. Results: The study showed for them in terms of the level of glucose, HbA1c, urea, and creatinine, as significant differences were found in the levels of these indicators as the duration of the infection increased. We also monitored the patients in terms of the type of treatment used during these periods and the diseases before and after diabetes. It was found that the longer the period of diabetes, the greater the risk of exposure to chronic diseases associated with diabetes. Conclusions: Through our study, it is clear that diabetes is a disease that develops in those infected with it, and this development of the disease leads to other complications that can affect the rest of the body’s organs, such as the eyes, heart, and kidneys.

Introduction: Staphylococcus aureus is recognized for its role in diverse clinical conditions, especially causing severe manifestations in patients with suppressed immune function. Methods: To evaluate bacterial presence, 24 clinical swab samples were systematically collected from suspected infection sites from patients in Najaf, Iraq. Bacterial culture showed positive growth in 15 samples 62.5 %, and 10 iso- lates were confirmed as S. aureus through macroscopy, microscopy, biochemical tests, and VITEK 2 identification. Results: Antibiotic susceptibility tests showed high rates of resistance, with 70 % of isolates resistant to ceftriaxone, followed by gentamicin, cephalosporin, erythromycin, and tetracycline, each at 60 %. Intermediate resistance was observed with chloramphenicol at 50 %, while the lowest rates were noted for rifampin and levofloxacin at 40 %. A significant proportion of the isolates 70 % exhibited resistance to multiple antimicrobial classes, while methicillin resistance was confirmed in 40 % of the S. aureus strains. When tested against these isolates, reduced graphene oxide (rGO) showed no inhibitory growth of S. aureus using the agar well diffusion assay. Conclusion: The findings highlight a pressing demand for innovative treatment approaches to address this growing resistance, given the increasing resistance to antibiotics and the failure of unmodified rGO to provide effective bac- terial inhibition under the conditions tested.

Objective: This review display the mechanistic link between inflammation response and male infertility disorders, emphasizing the immunological and oxidative ways contributed in the deterioration of sperm impotent and function. Method: The map of diffusion shows that pro-inflammatory cytokines overproduction and reactive oxygen species (ROS) production lead to oxidative stress, which results in cellular damage to membrane and nuclear structures, and subsequently causes apoptosis of sperm cells. Results: Male reproductive tract inflammatory conditions, including epididymitis and urethritis and varicocele diseases and testicular torsion, result in impaired sperm quality through their disruption of accessory gland function and sperm transport and spermatid formation process. With increased cytokine levels, sperm motility and morphology and concentration decrease. Novelty: Spermatogenesis is an elevated coordinated process involving the proliferation and differentiation of germinal cystocele within the testis in seminiferous tubules, essential for fertility in male specially. The complete process depends on hormonal control and epigenetic mechanisms, which make it vulnerable to different internal and external disturbances. Inflammation represents a major obstacle to spermatogenesis since it results from ,infections and anatomical abnormalities and environmental exposures

Background: The relationship between vitamin D levels as well as calcium consumption together with their impact on type 2 diabetes (T2D) development has become a subject of increasing scientific investigation during the past few years. Researchers have begun studying vitamin D as it extends beyond its established function to maintain bone health into multiple body systems which include its capacity to control glucose metabolism. The connection between these two elements operates through multiple channels which include insulin secretion control by pancreatic beta cells together with changes in peripheral tissue insulin sensitivity and the body's response to ongoing low-grade systemic inflammation. Multiple observational studies together with various detailed meta-analysis studies have found that higher levels of 25-hydroxyvitamin D in circulation help to decrease the chances of developing T2D. The evidence from randomized controlled trials which serve as the ultimate method to demonstrate causation remains unclear. The clinical trials which tested different vitamin D supplementation doses failed to show a constant protective effect against developing T2D which leaves the hypothesis without confirmation. Researchers still need to identify the 25-hydroxyvitamin D concentration which most effectively controls blood sugar levels and reduces diabetes development. Researchers need to establish these thresholds so they can convert observational research results into practical clinical guidelines.

Menopause and ovarian dysfunction are consequences of ovarian aging, a continuous and natural process in women. Early symptoms of ovarian aging include inadequate response to ovarian stimulation, irregular menstruation, and loss of follicular function, which collectively contribute to a decline in fertility as women age. The effects of aging on the ovaries are more pronounced than on other organs, with deterioration in the follicular pool and oocyte quality influenced by endocrine, genetic, and metabolic factors. This study aimed to examine the correlation between menopausal symptoms and serum levels of vitamin D, anti-Müllerian hormone (AMH), and other fertility-related hormones, as well as their impact on the timing of menopause. Ninety-two female participants were recruited from the Fertility Center Laboratories and AL-Saader Medical City in Al-Najaf province. A structured questionnaire assessing age, weight, height, and the severity of eleven menopausal symptoms—rated on a scale from 0 (absent) to 4 (extremely severe)—was administered. Symp toms evaluated included hot flushes, heart discomfort, sleep disturbances, muscle and joint pain, psychological issues, and urogenital problems. Serum levels of follicle-stimulating hormone (FSH), 25-hydroxyvitamin D, AMH, and luteinizing hormone (LH) were measured using the ELISA method. The results showed a signifi cant negative correlation between serum AMH and vitamin D levels with menopausal symptoms and advan cing age in postmenopausal women. Vitamin D levels were also negatively correlated with age, depression scores, and body mass index (BMI). Women with vitamin D deficiency and decreased AMH experienced more severe menopausal symptoms such as hot flushes, heart discomfort, depression, irritability, bladder problems, and musculoskeletal pain. These findings suggest that low AMH and vitamin D levels during menopause, compared to younger control women aged 20–40 years, may exacerbate menopausal symptoms. Supplementa tion with vitamin D, AMH, and reproductive hormone regulators, alongside lifestyle management, may benefit menopausal women suffering from fertility decline and severe menopausal symptoms

Cigarette smoking is a well-established disruptor of endocrine and reproductive function in males. This review synthesizes evidence from over 1,000 peer-reviewed studies, focusing on the impact of tobacco exposure on sex hormone profiles, sperm quality, and hypothalamic pituitary-gonadal (HPG) axis regulation. Despite paradoxically elevated levels of total testosterone observed in moderate and heavy smokers, increased sex hormone-binding globulin (SHBG) concentrations lead to reduced bioavailable testosterone, impairing androgen-dependent functions. Age-stratified analyses reveal transient elevations in young males and marked hormonal decline among older individuals due to oxidative damage and diminished Leydig cell reserve. Moreover, smokers exhibit significant declines in sperm count, motility, and DNA integrity, despite serum testosterone levels. Elevated SHBG and disrupted gonadotropin secretion further contribute to subfertility and endocrine dysfunction. Importantly, clinical trials confirm that smoking cessation can restore hormonal balance and improve reproductive outcomes within six months. These findings highlight the need for routine endocrine evaluation in male smokers and reinforce cessation efforts in the management of reproductive and metabolic health.

Epigenetics has emerged as a key field for understanding the interplay between genetic predisposition and environmental exposures in Human health. It is very unique because unlike with genetic mutations, epigenetics does not alter the genes themselves; rather it affects how the genes behave – whether they are turned on or off – offering us an amenable and reversible level of regulation. The induced alterations will act as molecular intermediaries of the gene-environment interactions that underlie susceptibility, course and phenotypic heterogeneity of complex human disorders. With increasing evidence suggesting that abnormal epigenetic changes are integrated into the development of cancer, cardiovascular diseases, neurological disorders, autoimmune diseases and metabolic syndromes. Nutrition, toxins, stress, and social determinants are environmental factors able to cause long-lasting epigenetic changes that can alter disease risk during the lifetime and even transgenerationally. This review summarizes our present knowledge on the mechanistic landscape underlying the epigenetic regulation process and its relevance in disease progression, as well as the possible use of precision/precision-like epigenetic therapies in practice, outlining the prospects and challenges to translating this understanding to precision medicine. Diet, toxins, stress and sociodemographic factors are the environmental aspects capable of producing persistent epigenetic modifications leading to change in disease risk both acutely throughout lifespan as well as intergenerationally. This review provides a general overview of the current mechanistic landscape of epigenetic regulation and how it relates to disease pathogenesis, as well as potential for precision/precision-like epigenetic therapies in clinical medicine, focusing on opportunities and challenges to implementing this understanding in practice.

The angiotensin-converting enzyme 2 (ACE2) is an enzyme of the reninangiotensin-aldosterone system (RAAS), which catalyzes the conversion of some other angiotensin. ACE2 is widely expressed in different tissues. Many researchers reported many microRNAsinvolved in ACE2 expression, hsa-mir181a one of these microRNAs which involved in ACE2 expression. In this study, we develop a comparative method (miR-Explore) to demonstrate that clustering of vaccinated miRNAs hsa-mir-181a of gene ACE2- alters the sensitivity and specificity of the prediction method even when a simple direct alignment of the positional secondary structure is used. The basic idea of the current approach was to generate a predictive visualization of the miRNAs hsa-mir-181a consensus structure for each vaccinated and unvaccinated miRNAs hsa-mir-181a class and use this consensus structure to perform alignment with the query sequence A set of data was taken from premiRNA by extracting total RNA and converting it to cDNA, then reading the DNA sequence and following up the in situ secondary structure of these sequences by means of dedicated software programs and websites and comparing them with each other and with sequences available in the microRNA database website.

Respiratory tract infections RTIs represent a major global health concern, contributing to significant morbidity and mortality, particularly in low- and middle-income countries LMICs. The increasing burden of antimicrobial resistance AMR has further complicated the management of RTIs, limiting available treatment options and increasing healthcare costs. Smoking has been widely recognized as a significant risk factor for RTIs, compromising pulmonary immune defenses and promoting chronic colonization by multidrug-resistant MDR pathogens. However, limited data exist on the interplay between smoking and AMR trends in Iraq, a country experiencing rising antibiotic resistance rates due to unregulated antibiotic use and healthcare disruptions.
