Abstract

Abstract Infection is presumed to have a rule in the promotion of asthma exacerbations, and in deterioration of the course of the disease, Chlamydia pneumoniae (C. pneumoniae) is claimed to be a possible cause for these two issues. To assess the positivity and the titer of C. pneumoniae IgG antibodies in relation to the state of asthma and its severity. 61 asthmatic patients aged 15-85 years (mean of 47.10±14.887), and 29 apparently healthy, nonasthmatic age and gender-matched volunteers (control group) were assessed as at Asthma and Allergy Clinic in Al-Sader Medical City in AL-Najaf province, Chlamydia Pneumoniae IgG ELISA Kit was used for the detection of IgG antibody to C. Pneumoniae in human serum to detect chronic infection, and Spirometric test was done, and the best results for FEV1 and PEFR were taken. IgG antichlamydial antibodies were positive in 21 (34.4%) of patients compared to 4 (13.8%) of controls, and the difference was significant with OR=3.281, the seropositivity in acute exacerbation was more than in stable asthmatic, 43.8% vs. 24.1% (P=0.029), seropositivity was nonsignificantly more in moderate and severe asthma as compared with mild asthma, a significant inverse correlation between IgG titer and pulmonary function test parameters (FEV1, PEFR) was observed as the FEV1 & PEFR values decrease with increase IgG titer. Chronic C. pneumoniae infection is common in adult asthmatics and correlated with exacerbations & increased severity and disturbed lung function.

Abstract

Background: Asthma is an important community problem that increased in incidence worldwide, with a considerable acute and chronic sequale and being an inflammatory process, following and interrupting this process is extremely important. High‑sensitivity C‑reactive protein (hs‑CRP) is proposed to be a good indicator for the degree of inflammation; we aimed to utilize hs‑CRP as an inflammatory biomarker to assess the severity and stability of asthma. Patients and Methods: Forty-one asthmatics (19 males and 24 females) with a mean age of 41 ± 14 and 22 years and gender- and body mass index-matched controls were assessed by questionnaire, spirometry, and level of hs-CRP in Al‑Sader Teaching Hospital in Al‑Najaf at 2018. Results: The mean values of hs‑CRP were significantly higher in asthma than the control group, and this was marked in asthmatics during acute exacerbation than those with stable clinical status, hs‑CRP was recorded significantly higher in adult asthmatic patients as compared to adolescents and middle aged, smoking history and allergic history did not show any significant difference, and the elevation of hs‑CRP was markedly more in acute exacerbation as compared to stable state; on the other hand, the level of hs‑CRP was correlated significantly more with the drop in forced expiratory volume1 and peak expiratory flow rate, indicating the relation with the severity of the illness. Conclusions: Hs‑CRP can be considered as a good biomarker for assessing the severity and stability of asthma

Abstract

Enterobiasis is the commonest intestinal helminthic disease and is long believed to be a simple illness with no important local or systemic invasive complications. A cross-sectional analytic study of 175 patients attending the surgical clinics with complaints of perianal lesions like perianal fissure, perianal fistula, perianal abscess and perianal pruritus and excoriation. Forty two (93%) of the patients in the first group had a low serum zinc (mean 7.27umol/l), this was significantly lower than the second group and the two control groups (patients with uncomplicated Enterobiasis and a normal control group), however, the mean serum level was lower than normal in these groups. After treatment of the first group with two doses of Mebendazole (100mg) at two weeks interval, 8 patients (18%) had complete cure of their perianal pathology (two anterior perianal fissure and eight pruritus and excoriation), 10 patients (22%) had a moderate response and 13 patients (29%) had a mild response. Out of 175 patients, 45 patients had enterobiasis on twice scotch tape tests. There were 34 (76%) males and 11(24%) female positive cases of enterobiasis. Perianal fissure was the most common lesion in both groups: 30 (67%) in the first group versus 96 (75%) in the second group. Perianal pruritus with excoriation occurred in 14 cases (31%) in the first group and 16(12%) in the second. Perianal fistula and perianal abscess occurred in 5(11%) and 3(6%) in the first group and in 26(20%) and 24(18%) in the second group respectively

Abstract

Chronic obstructive pulmonary disease (COPD), a group of diseases of distinct aetio-pathological consideration with different phenotypic presentations where smoking is the leading cause, all share the ultimate result of airflow limitation. This study aimed to evaluate thyroid function tests (TFT) in patients with COPD. Pulmonary function tests (PFT) were performed for 30 patients with obstructive lung disease and fifteen healthy control individuals. We measured SPO2 to confirm COPD and assess the severity of the disease and assessed TT3, TT4, and TSH using the ELISA test. The values of VC, FVC, and FEV in the first second and PEF, TSH, and SPO2 were lower in the COPD group than in the control group (P-value=0.001). In severe COPD (FEV1 <50%), there was a significant re duction in T3 but not T4 or TSH compared to mild-moderate COPD patients. Thyroid dysfunction was observed in patients with COPD pointing to a metabolic response; patients with lower weight indices had a lower TSH and, consequently, T3

Abstract

Background: Coronary artery disease (CAD) is a group of diseases caused by plaque formation in the wall of arteries supplying blood to the heart. Adiponectin (ADIPOQ) is an adipocytokine that controls carbohydrate and lipid metabolism, has anti-diabetic properties, and correlates well with cardio-metabolic risk. ADIPOQ gene polymorphism is reported to be associated with CAD in numerous populations. Objective: To explore the impact of adiponectin gene polymorphism, rs2241766 T > G, rs1501299 G > T, rs17300539 G > A, rs266729 G > A, and rs822395 A > C SNPs, on the CAD in the Iraqi population. Methods: A case-control study of 317 healthy individuals and 302 CAD patients was conducted. Standard pro tocols for measuring serum lipid profile levels were followed. ADIPOQ and insulin levels were estimated using the ELISA method. Genotyping of ADIPOQ gene was carried out using the RFLP technique. Results: Genotype assessment of rs2241766, rs1501299, rs266729, and rs17300539 SNPs under several inheri tance models pointed out significant (P = 0.012–10 4 ) elevations of the variant alleles in CAD patients in comparison to the control group. The haplotype analysis of the five analyzed SNPs exhibited five disease susceptible haplotypes; they were GTCGA (OR: 7.6, 95 % CI: 2.55–21.76), TGCAA (P = 0.013), TGGGA (P = 0.046), TGGAA (P = 0.017), and GGGGA (P = 0.032). The linkage disequilibrium (LD) analysis of the five SNPs in the control versus the patient groups revealed significant LD. The variant genotype of the rs2241766 SNP was found to significantly increase the atherogenic lipids and HOMA values and decrease the ADIPOQ and high density cholesterol (HDL-c) levels. Those of the rs1501299, rs17300539, and rs266729 SNPs were indicated to increase the metabolic parameter values except ADIPOQ and HDL-c magnitudes, which are reduced in the variant carriers. Conclusion: The rs2241766, rs1501299, rs266729, and rs17300539 SNPs of the ADIPOQ gene are risk factors for CADin Iraqi individuals. Both single nucleotide and haplotype analyses suggested the implication of the ADIPOQ gene in the pathogenesis of the disease. This implication has occurred through lowering ADIPOQ levels and increasing atherogenic lipids

Abstract

Evaluation of IL-39 serum level in Iraqi diabetic patients was the aim of this research. A case control study, 60 type 2 diabetic patients matched with 50 non-diabetic controls The results revealed a substantially high levels of IL-39 among diabetic patients in comparison to control (87.25±10.24 vs. 64.14±2.07, p=0.03). Statistical inference of IL-39 serum level show that patients with poor glycemic control and obesity show higher levels than those with good glycemic control and non-obese and patients with cardiovascular complications show high levels in comparison to those without this issue(104.25±23.52 vs. 75.11±4.79, p=0.2). IL-39 elevated in type 2 diabetic reflecting its possible role in disease pathogenesis, it was not associated with demographic, clinical and cardiovascular complications. Abstract

ABSTRACT Background: Increasing value of Red cell distribution width (RDW) is documented to be linked to worse clinical outcomes so that it can be considered as a prognostic indicator in states like acute myocardial infarction coupled with other inflammatory markers taking in consideration its easy availability. Aim of the study: This study aimed to assess the relationship of RDW and echocardiographic parameters especially systolic and diastolic dysfunction in patients with ST-elevation myocardial infarction (STEMI). Patients and methods: In this study 60 {38 (63%) were males and 22 (37%) females} consecutive patients who had acute ST elevation myocardial infarction (STEMI) at Rozhalat hospital, Erbil, Iraq were included, RDW was obtained during the event of STEMI and the levels obtained where analyzed according to measures of systolic and diastolic dysfunction calculated by transthoracic echocardiophic study. Results: RDW was found to be increasing in patients with impaired systolic and diastolic functions (ejection fraction of the left ventricle <50%, diastolic filling wave (E/A)<1 , peak early transmitral velocity over early diastolic annular velocity [E/E'] >10 , and more obviously with combined E/A < 1 and E/E' > 10 (P <0.01). The best cut off value of RDW was 13% with sensitivity (75.9%) and specificity (61.3%) and this is obtained applying ROC curve analysis according to echocardiography ejection fraction 50%. Conclusion: Elevated RDW can be considered as a new biomarker of poor outcome in patients acute STEMI patients as it is associated with diastolic as well as and to a lesser extent systolic dysfunction in patients with STEMI even after adjustment of several confounding factors.
