المجلةAsian Spine Journal
مختصر البحث

Study Design: Cross-sectional retrospective study designed to assess interobserver agreement. Purpose: To investigate if interobserver agreement using magnetic resonance imaging (MRI) in the evaluation of lumbar spinal canal stenosis and root compression can be improved upon combination with magnetic resonance myelography (MRM). Overview of Literature: The interpretation of lumbar spinal MRI, which is the imaging modality of choice, often has a significant influence on the diagnosis and treatment of low back pain. However, using MRI alone, substantial interobserver variability has been reported in the evaluation of lumbar spinal canal stenosis and nerve root compression. Methods: Hardcopies of 30 lumbar spinal MRI (containing a total of 150 disk levels) as well as MRM films were separately reviewed by two radiologists and a neurosurgeon. At each intervertebral disk, the observers were asked to evaluate the thecal sac for the presence and degree of spinal stenoses (mild, moderate, or severe) and presence of root canal compression. Interobserver agreement was measured using weighted kappa statistics. Results: Regarding lumbar spinal canal stenosis, interobserver agreement between the two radiologists was moderate (kappa, 0.4) for MRI and good (kappa, 0.6) for combination with MRM. However, the agreement between the radiologist and neurosurgeon remained fair for MRI alone or in combination with MRM (kappa, 0.38 and 033, respectively). In the evaluation of nerve root compression, interobserver agreement between the radiologists improved from moderate (kappa, 0.57) for MRI to good (kappa, 0.73) after combination with MRM; moderate agreement between the radiologist and neurosurgeon was noted for both MRI alone and after combination with MRM (kappa, 0.58 and 0.56, respectively). Conclusions: Interobserver agreement in the evaluation of lumbar spinal canal stenosis and root compression between the radiologists improved when MRM was combined with MRI, relative to MRI alone.

المجلةThe Iraqi Postgraduate Medical Journal
مختصر البحث

ABSTRACT: BACKGROUND: Vertebro-basilar dolichoectasia (VBD) is an uncommon vasculopathy of unclear origin affecting the arterial wall of vertebral artery (VA) and basilar artery (BA). A variety of clinical syndromes and neuroimaging features have been associated with VBD. OBJECTIVE: This cross sectional study is conducted to shed a light on the neuroimaging signs of VBD and explore the association between the clinical presentation and imaging parameters of VBD in patients clinically suspected to have VBD with neuroimaging diagnosis. MATERIAL AND METHODS: This randomized cross sectional study was performed in Middle Euphrates Neuroscience Centre in Annajaf city between April 2012 to December 2012,A 34 patients with neuroimaging diagnosis of VBD (18 patients by MRI and 16 patients by MDCT) were included, VBD were assessed by studying the basilar artery width (BAW), bending length (BL) and level of basilar artery bifurcation according to Dan Deng et al criteria. The findings were correlated with patient clinical presentation. RESULTS: Thirty four patients, (24 males & 10 females) with age range between 25-81 years and mean of 57.9 year were included in this study. Among all presenting symptoms, only headache showed statistically significant association (p value <0.05) with the width of the basilar artery. There was higher prevalence of single cranial nerve involvement (SCNI) in patients with more tortuous basilar artery (55.5% of those with BL more or equal to 10 mm versus 16% of those less than 10 mm) with statistically significant association (p value <0.05), suprasellar extension was seen in 26.4% of cases and showed significant association with headache and visual symptoms. CONCLUSION: \VBD is an important clinical entity and could be associated with or responsible for many serious clinical presentations, special attention is needed for VBD diagnosis and it's parameter during evaluation of brain imaging especially in patients with un-explained or vague presentation. KEYWORDS: vertebrobasilar, dolichoectasia.

مختصر البحث

Abstract; Background: Stroke remains one of the leading cause of the death in the world and despite the advance in neuroimaging, computerized tomography (CT) is still widely used in evaluation of patients with ischemic stroke in emergency unit which should be done before giving thrombolytic therapy in the first 3-4.5 hour of stroke onset .Aim of this study is to evaluate early brain CT findings among ischemic stroke which is of great help in deciding early stroke management. Patients and methods: A cross-sectional descriptive study was done at Middle Euphrates Neuroscience center; AL-Sadder Medical City at Annajaf city – Iraq, in the period from may 2012 to December 2014. A total of 108 patients whom clinically diagnosed as stroke were referred for CT scan of brain within 48 hours of the stroke onset. Twelve patients with intracranial hemorrhages were excluded and the remaining 96 patients were studied for early brain CT findings. Results: The prevalence of ischemic infarction was 85%, with slight male predominance (male: female ratio=1.25:1) and most affected patients with ischemic type were in range of 60-69 years. The sensitivity of CT scan in detection of the ischemic changes increased linearly with time from onset of stroke. The prevalence of the hyperacute signs of middle cerebral artery infarction (insular- ribbon sign and hypodense basal ganglia) in acute ischemic stroke were seen in 33.3%, no case reported to show hyper dense middle cerebral artery sign in our study. Conclusion: CT remains the initial imaging modality of choice for the early diagnosis and management of acute stroke especially in making the diagnosis within the therapeutic window for thrombolytic therapy where the time is crucial for detectability. Specific signs of hyperacute infarction should be carefully scrutinized for early detection. Key wards: Stroke, brain, computerized tomography.

المجلةJournal of kerbala university
مختصر البحث

Abstract Background:Based on physical examination alone, it is often difficult to decide whether a palpable scrotal swelling arises from testicle itself or from extra testicular elements in scrotum. Aim of the study: To evaluate the causes of scrotal swellings in different age groups using sonography.Patients and method:One hundred patients, with age ranging between1-77years(mean age of 31 +/- 7 years), presented with scrotal swellings were examined by real-time ultrasound during the period from October 2011 to April 2012 in Al-Sadir Medical City at Al-Najaf government-IraqResults: Sonographic examination was normal (could not detect any lesion) in 7 patients (7%). The commonest sonographic finding was hydrocele (33.1%) while the next common findings were varicocele (20.3%) and epididymal cysts (14.4%). Inflammatory causes were detected in 14 patients (11.8%). Of the 8 (6.7%) patients with scrotal trauma, 5 cases showed features of hematocele and 3 cases extra-testicular hematoma. Least sonographic findings were testicular torsion, scrotal hernia and solid intratesticular mass (2.6% for each). Most of scrotal swellings were unilateral (right more than left) and 8% showed contralateral incidental findings. Conclusion: Majority of causes of scrotal swellings were benign with the hydrocele being the commonest cause among all age groups. The least common causes of scrotal swellings were scrotal mass, hernia and testicular torsion. Although most of causes of scrotal swellings were benign, some were serious requiring urgent intervention, particularly testicular torsion, necessitating no delay in ultrasound examination for evaluation of patients with acute scrotal pain and swelling. To overcome limitations of this study, future study encompassing larger sample, including emergency cases, is recommended.

Abstract: Modic endplates signal changes as seen by magnetic resonance imaging (MRI) of lumbar spine are well known, however their clinical relevance and role in disc herniation is not fully understood. This study aims to evaluate the relationship between presence of Modic changes and herniation & to correlate severity of both.Retrospective study of MRI examinations of 57 patients (36 males, 21 females) with 160 herniated lumbar discs was done regarding severity of herniation and presence of Modic changes. Characteristic of Modic changes including type, side, extent & depth were further assessed and correlated with degrees of herniation. Prevalence of Modic changes in herniated discs was 27.5% with significant association (P value <0.05) with extrusion. Extrusion showed significant association with type-II, sever extent, both sides involvement and grade II-III depth (all P values were <0.05). The more severe herniation of the lumbar disc is, the more prevalent and more severe are the Modic changes, suggesting a positive relationship, however further clarifying studies are still recommended.

مختصر البحث

Abstract The widespread and rapidly increasing use of computerize tomography (CT) has raised the burden of radiation to patients with consequent raise of risk of radiationinduced cancer. Objective: to assess patient's awareness of the risk of cancer from CT radiation and the level of patient's informing. Methods: A questionnaire was administrated to 100 patients (55 female, 45 male) who were referred to CT unit. Results: Majority of patients (86%) have no awareness of the high radiation implied by CT examinations and the increased risk of cancer. Sixty three (63%) of referred patients had not been informed about risk neither by referring physician, radiologist nor by the CT technologist, however 40 of them (63.5%) would accept examination, 15 (23%) would refuse it and 8 (12.7%) would hesitate if they had been told about that risk. Conclusion: There is great deficiency in the patient's knowledge of the risk from CT radiation and majority are not informed by medical personnel. Urgent actions and programs, specially implicating media, to reasonably educate patient about these issues seems necessary. Further larger-sample studies to assess awareness of patients as well as medical staff are recommended.

الباحثونHaider Najim Aubaid
المجلةKufa Med.Journal
مختصر البحث

ABSTRACT The nature of relationship between vertebral endplate signal changes (VESC) with lumbar intervertebral disc (IVD) degeneration is still relatively controversial. Aim of study is to evaluate VESC in lumbar spine and correlate their characteristics with IVD degeneration. MRI examinations of 60 patients (38 males, 22 females) with a total of 300 IVD were studied retrospectively. IVD degeneration was graded according to system modified by Pfirrmann et al. VESC were assessed regarding type, extent and depth and then correlated with grades of degeneration at affected levels. VESC were most frequent near lumbar IVD that had advanced degeneration (43.5% in grade IV and 39.1% in grade V) with statistically significant association (P value < 0.05). Higher grades of IVD degeneration showed significant association (P value <0.05) with type II, sever extent, and grade I-II depth of VESC. As conclusion, VESC seem to be positively related to the lumbar IVD degeneration with more severe characteristics of VESC, particularly extent, are linked to more advanced degeneration, however further clarifying studies are still recommended

الباحثونHaider Najim Aubaid ، Hayder Qassim El Mosawi Hayder K. Hussein
المجلةMedical Journal of Babylon
مختصر البحث

Abstract Background: Magnetic resonances imaging (MRI) is reliable in evaluation of lumbar disc degeneration, however there is a variation in grading of degeneration. Aim of the study is to measure observer variability in assessing degeneration using a grading system. Patients and methods: Tow radiologist assessed degeneration of 150 lumbar discs of 30 patients (21 male & 9 female) by evaluating MRI examinations, using a grading system after some modifications in nomenclature. Intraobserver and interobserver agreements were measured using weighted Kappa statistic. Results: Intraobserver agreement ranged from moderate (kappa of 0.55) to very good (kappa of 0.87) while interobserver agreement ranged from moderate (Kappa 0.46) to good (kappa of 0.76). Main disagreement were between severe and moderate grads of degeneration while best consensus was obtained in normal discs. Conclusion: Moderate to very good agreement obtained in application of our grading system. Disagreement may be reduced by proper and more frequent use & increasing familiarity. However, further studies with larger sample and among more observers may be still required for the system to be practically applicable.
