Coronavirus disease 2019 (COVID-19), caused by SARS-CoV-2, induces systemic immune dysregulation with elevated inflammatory biomarkers such as C-reactive protein (CRP), ferritin, and D-dimer. The interaction between CXCL3-mediated airway inflammation and systemic inflammatory responses in asthmatic patients during COVID-19 infection remains insufficiently elucidated, with limited data clarifying its contribution to disease severity and outcomes. Objective: To evaluate the correlation of serum CXCL3 and classical inflammatory markers (CRP, ferritin, D-dimer) with asthma severity among patients with and without COVID-19 infection and to determine their relationship with lung function (FEV₁) and predictors of severe asthma. Methods: A cross-sectional study was conducted on 200 asthmatic patients, stratified according to COVID-19 infection status (76 positive, 124 negative). Demographic, clinical, and laboratory parameters were analyzed. Serum levels of CXCL3 and inflammatory markers (CRP, ferritin, and D-dimer) were compared between groups and correlated with asthma severity and FEV₁. Results: COVID-19-positive asthmatics exhibited significantly higher levels of CXCL3 (241.5 ± 83.7 vs. 173.8 ± 76.4 ng/mL, p<0.001) and inflammatory markers, along with lower FEV₁ (54.3 ± 17.2 vs. 62.1 ± 18.9; p=0.003) compared to COVID-19-negative patients. CXCL3, CRP, and D-dimer levels increased progressively with asthma severity (p<0.001), furthermore, significant positive correlations were observed between CXCL3 and asthma severity (r = 0.62) as well as between CXCL3 and CRP (r = 0.41). Logistic regression revealed FEV₁ <60% as an independent predictor of severe asthma (OR 3.12, 95% CI 1.78–5.47, p<0.001), whereas COVID-19 infection showed an inverse association with severe asthma (OR 0.48, 95% CI 0.25–0.91, p=0.026). Conclusion: Serum CXCL3 and systemic inflammatory markers are significantly associated with asthma severity and lung function impairment, particularly in the presence of COVID-19 infection. CXCL3 may serve as a potential biomarker for assessing asthma severity and guiding clinical management in patients with concurrent COVID-19. Objectives: This study … Background: New rheumatoid arthritis (RA) biomarkers, such as serum interleukin-37, may aid in the identification and management as well as improved monitoring of disease severity, activity, and effectiveness of the therapy. Objectives: This study aims to assess the association between gene expressions of interleukin (IL)-37 isoform-a and isoform-b and serum IL-37 levels with the severity and activity of RA. Materials and Methods: This is a case–control study of 140 participants. The samples were collected using fresh blood and serum samples. The sample consisted of 64 females and six males in both the patient and control groups. The serum IL-37 level was measured using enzyme-linked immunosorbent assay. Immune markers such as anti-cyclic citrullinated peptide (anti-CCP), erythrocyte sedimentation rate (ESR), rheumatoid factor, and C-reactive protein (CRP) were also measured. Gene expression for IL-37 isoforms was measured by real-time quantitative polymerase chain reaction (PCR). DAS- 28-ESR, DAS-28-CRP, and CDAI were used to calculate the activity of RA as well as the severity of disease. Results: The serum levels of IL-37 were significantly higher in patients than in controls. Activity parameters, as stated by DAS-28 ESR, DAS-28 CRP, CDAI, and SDAI, showed a positive correlation with serum levels of IL-37. The levels of IL-37 correlated significantly with the treatment response. There was a significant correlation between IL-37 levels and ESR, CRP, and anti-CCP levels. IL-37 isoform-a mRNA expression, measured by RT-qPCR, was upregulated in patients with severity. In addition, the isoform-b gene expression was downregulated, in contrast to control groups for both genes, who showed normal expression for these genes. Conclusion: Serum IL-37 and IL-37 isoform-a are promising biomarkers for the diagnosis of RA, assessing disease activity and severity in patients with RA, and offering possible future therapeutic applications for RA patients, while IL-37 isoform-b is considered a weak biomarker for diagnosing RA. Patients suffer from infection with SARS-Covid-2 had measurements of f… ABSTRACT Background: The pandemic results of Coronavirus 2019 (COVID-19) range from moderate to serious, necessitating the rapid assessment and association of diagnostic markers. Patients suffer from infection with SARS-Covid-2 had measurements of ferritin, LDH, and D-dimer. In addition to being a problem for healthcare institutions, the SARS-Covid-2 additionally represents a major financial burden. Aim: To assess the predictive importance of serum ferritin & D-dimer levels as predictors for individuals at risk of serious and fatal infection with SARS-Covid-2. Methods: This study designs as Case-control study was done at AL-Amal Specialized Hospital for Communicable Diseases in Al-Najaf, Iraq, during the period of June to August of 2023. Prior to their enrollment in the clinical study, every individual participant provided written consent. 44 samples which included 22 patients (8 men and 14 women) and 20 control (8 men and 14 women) and varying age groups from 28 to 76 years old—were reported to have symptoms of SARS-Covid-2 infection. Results: The present investigation reveals significant differences between the patient and control groups, as well as between the patient and control groups' age groups and sexes. Patients who are older have a higher age-related prevalence of severe SARS-Covid-2 infection. In total, 22 COVID-19 patients were incorporated into this research; 8 (36.4%) were male and 14 (63.6%) were female. In comparison, there were 8 (36.4%) male and 14 (63.6%) female among 22 healthy control individuals. According to the statistical analysis, there were statistically significant differences betweenthe female and male patients. There wasn't a noticeable difference in the mean serum level of ferritin between the SARS-Covid-2 infected patients' group and the control group (μg/l). Conversely, it was highly significant difference in mean serum level of D-Dimer between the COVID-19 patient group and the control group (μg/l). Conclusion: The age-specific prevalence of severe SARS-Covid-2 infections increases in older adults age of patients: Females showed higher rates of infection than males. Higher level Serum of D-dimer and ferritin are linked with outcomes in patientsCovid-19. Significantly elevated ferritin and D-dimer levels in infection with SARS-Covid-2 recovered individuals, which may serve as SARS-Covid-2 diagnostic biomarkers.
