Diabetes mellitus associated chronic kidney disease (diabetic renal disease) became the predominant etiology of the end-stage renal disease (ESRD). In this study we have explored the possible role of IL-6 as biomarkers of inflammation in diabetic and non-diabetic ESRD patients. The study population was included 50 diabetic and 30 non-diabetic ESRD patients. All of them were on regular hemodialysis. The clinical assessment revealed that the frequencies of distribution of patients according to gender were statistically not significant. Assessment of age presentation of diabetic ESRD patients revealed that their ages ranged from 26 to 75 years, with a mean age of 54.45 years. The estimated incidence of ESRD in diabetic patients increased in the age group 56-65 years, with a significant difference (p<0.05) in comparison with the other groups. RFLP-PCR technique was performed for detection the diagnostic prevalence and prognostic significance of polymorphism - G 174C in the promoter regions of IL-6 in diabetic and non-diabetic ESRD. The genotype distribution of - G174C SNP in the promoter regions of IL-6 showed significant difference (p<0.05) between diabetic patients (GG: n= 38, 76%; GC: n=10, 20%; CC: n=2, 4%) and non-diabetic patients (GG: n= 28, 93.33%; GC: n=2, 6.67%; CC: n=0). Both diabetic and non-diabetic ESRD patients were exposed for the immunological study represented by estimating the concentration of IL-6 by ELISA technique. The study results showed that the mean serum concentrations of IL-6 in diabetic patients were significantly (p< 0.05) higher than their mean serum concentrations in non-diabetic ESRD patients. The association of IL-6 serum levels according to genotypes of polymorphism recorded that serum level was higher in diabetic ESRD patient than non- diabetic patients. Generally IL-6 serum level was statistically higher in carriers of CC or GC than GG in ESRD patients.
