Ki-67 and P53 are two cellular proteins playing roles in pathogenesis and e… Abstract: Background: The most common primary CNS neoplasms are astrocytomas, derived from an immortalized astrocyte, accounting for 4-5 % of all cancer related death. Ki-67 and P53 are two cellular proteins playing roles in pathogenesis and evolution of astrocytomas.Ki-67 is an antigen represents a nuclear non histone protein, produced by cells in the proliferative phases. P53 gene produces a protein functions as a tumor suppressor gene. Both biomarkers were considered to be of prognostic value. Aims: The paper aims theassessment of p53 and ki-67 immunoexpression in astrocytomas,relation of these markers with histologic grade of astrocytomas, andto use p53 and Ki-67 as a prognostic tool. Materials and Methods:Forty-five patients with astrocytoma were conducted in the study which performed in Department of Pathology, Faculty of Medicine, university of Kufa in the period from January 2018 through April 2019.Cases were collected from “neurosurgical hospital in Baghdad, and laboratory of histopathology in Al-sadder Medical city in Al-Najaf and from some private laboratories of the same governorate”, ages ranging from 2-65 years with a mean of 29.04 years.EnVision method (polymer based immunohistochemistry) was used for evaluation of p53 and ki-67. Results: Grade II patients predominant the studied sample and constitute (55.6%), whereas patients of grade I constitute (22.2%), grades IV and III compose (13.3%) and (8.9%) respectively.P53 was detected in (33.34%) of the cases and was “significantly positively correlated” with grade IV, gender not with age.Ki-67 LI was (>5%) in (31.11%) of patients and was “significantly positively correlated” with tumor grade, age of the patient but not with gender, both biomarkers were “positively correlated with each other, and with grade of astrocytoma”. Conclusions and recommendations:p53 and ki-67 over expression have an important role in pathogenesis of astrocytoma, as their positivity associated with higher tumor grade, so merging these biomarkers with histopathological grade in a prognostic index will accurately predict clinical outcome and the effectiveness of anti cancer therapy. ABSTRACT— Left ventricular dysfunction (LVD) is an important complication of coronary artery disease (CAD) and represents a powerful determinant of survival. the purpose of the current study was to assess scar tissue for patient with old myocardia… ABSTRACT— Left ventricular dysfunction (LVD) is an important complication of coronary artery disease (CAD) and represents a powerful determinant of survival. the purpose of the current study was to assess scar tissue for patient with old myocardial infarction and LV systolic dysfunction by speckle tracking longitudinal strain using regional and global longitudinal strain and to compare it with late enhancement by CMR which is the gold standard method for assessment of scar transmurality, scar transmurality was the predictor of myocardial viability, the current study defined regional longitudinal strain cutoff value that discriminated between viable and non-viable myocardium. Methods: Thirty-eight patients with old myocardial infarction and LV systolic dysfunction from AL-Najaf cardiac center and AL-Nasirya cardiac center underwent transthoracic echocardiography to measure global and segmental (regional) longitudinal LV strain using two-dimensional speckle tracking and cine MRI followed by contrast-enhanced MRI to assess segmental LV function and the segmental/global (transmural) extent of scar tissue. The optimal cutoff value for regional strain to discriminate between segments with viable myocardium and segments with transmural scar was also determined. Results: A good correlation was found between global LV strain and the global extent of scar tissue on contrast-enhanced MRI (R= 0.518, n = 38, p = 0.001.). The mean segmental strain in segments with a scar score of 3 was -6.33% ± 2.28%, and for segments with a scar score of 4 was -2.67% ± 3.69%. A strain value of the cutoff value of segmental strain was calculated using (ROC curve) analysis, and it was found to be -7.5% discriminated between segments with viable myocardium and segments with transmural scar tissue on contrast-enhanced MRI with a sensitivity of 91.5% and specificity of 89.7% Conclusion: global and regional longitudinal strain measured with two-dimensional speckle tracking miscorrelated with the global and regional extent of scar transmurality assessed by contrast enhanced MRI. A cutoff value of -7.5% for regional strain discriminated between segments with viable myocardium and those with transmural scar tissue on contrast-enhanced MRI with a sensitivity of 91.5% and specificity of 89.7%.

In spite of its increasing incidence, morbidity and mortality are starting to fall; this may belong to evolving ways of diagnosis and treatment. P53 acts as a transcription factor, regulates cell cycle, and hence functions as a tumor suppressor gene. The most potent endothelial cell Mitogen is VEGF emerged as a prognostic marker in patients with several types of cancer including breast cancer. Both markers were proved to be of prognostic value, moreover simultaneous expression of p53 and VEGF has been reported to have the worst bio-pathological profile in breast carcinoma. Although predictive value of the above biomarkers had been well known in previous investigations, relation between these biomarkers in breast cancer is a point to be discussed more specially in our country. Aims: To evaluate the rate of p53 and VEGF immunoexpression in different clincopathological parameters of breast carcinoma, their correlation and co expression in Iraqi females. Materials and Methods: Seventy two breast cancer patients were included to this study which performed in Pathology Department, Faculty of Medicine, University of Kufa, from October 2018 to May 2019. All the cases were collected from Al-Sadder medical city and some of private laboratories in Najaf, they ranging age between22-70 years with a mean of 46.9 years. (LSAB+ ) method was used for IHC detection of p53 and VEGF. Results: Sixty two (86.11%) of the cases were of ductal type while 10 cases (13.89%) of lobular type. The detection rate of p53 and VEGF was 36.11% and 58.33 respectively. P53 immunoexpression was significantly different among tumor sizes, but not in other parameters. However, p53 correlated positively with tumor grade and size. There was a significant difference of VEGF immunoexpression among the above histological subtypes, but not within different tumor sizes, grades and age of the patients, meanwhile VEGF was positively correlated with tumor grade and size. Moreover, both biomarkers were positively correlated with each other, and cases co expressing both p53 and VEGF (19.44%) have shown the most unfavorable bio-pathological profile. Conclusions and recommendations: P53 and VEGF immunoexpression have an important role in pathogenesis of breast carcinoma evolution, and their positivity associated with biologically aggressive tumors, thus recommendation for larger prospective study will give better insight about survival and outcomes.

Immunohistochemical Expression of VEGF in Relation to Other Pathological Parameters of Breast Carcinoma

More recently, the expression of vascular endothelial growth factor (VEGF), the most potent endothelial cell mitogen and also a regulator of vascular permeability, is emerging as a prognostic marker in patients with several types of cancer including breast cancer. This study assessed the expression of VEGF in a series of breast cancers in correlation with HER-2/neu and steroid receptors (ER and PR) in standard clinicopathological parameters in an attempt to clarify its potential clinical importance in Iraqi females of Middle Euphrates area. Findings: The present investigation was performed over a period starting from September 2011 through September 2012. Formalin-fixed, paraffin-embedded blocks from 52 patients with breast cancer (44 ductal and 8 lobular carcinoma) were included in this study. A group of 20 patients with fibroadenoma was included as a comparative group, and 20 samples of normal breast tissue sections were used as controls. Labeled streptavidin-biotin (LSAB+) complex method was employed for immunohistochemical detection of VEGF, HER-2/neu, ER and PR. The detection rate of VEGF, HER-2/neu, ER and PR was 59.62%, 36.96%, 34.62% and 36.54% respectively. There was a significant difference in immunoexpression between ductal and lobular carcinoma, but not significantly different among tumor sizes, tumor grades, axillary lymph node involvement and age of the patients. However, VEGF was positively correlated with tumor grade, tumor size, nodal involvement and HER-2/neu, but negatively correlated with ER and PR, which show the most unfavorable biopathological profile. Conclusion: VEGF overexpression play an important role in pathogenesis of breast carcinoma evolution, as its positivity associated with biologically aggressive tumors, so incorporation of this biomarker with other parameters into a prognostic index will more accurately predict clinical outcome and determine the effects of anti cancer therapy. This study looked at the immunohistochemical expression of HER-2/neu (c-erbB2) and p53 in different histological types of breast cancer found in the middle Euphrates region of Iraq, where the population has been exposed to high levels of depleted uranium. FINDINGS: The present investigation was performed over a period starting from September 2008 to April 2009. Formalin-fixed, paraffin-embedded blocks from 70 patients with breast cancer (62 ductal and 8 lobular carcinoma) were included in this study. A group of 25 patients with fibroadenoma was included as a comparative group, and 20 samples of normal breast tissue sections were used as controls. Labeled streptavidin-biotin (LSAB+) complex method was employed for immunohistochemical detection of HER-2/neu and p53.The detection rate of HER-2/neu and p53 immunohistochemical expression were 47.14% and 35.71% respectively in malignant tumors; expression was negative in the comparative and control groups (p < 0.05).HER-2/neu immunostaining was significantly associated with histological type, tumor size, nodal involvement, and recurrence of breast carcinoma (p < 0.05), p53 immunostaining was significantly associated with tumor size, nodal involvement and recurrence of breast cancer (p < 0.05). There was greater immunoexpression of HER-2/neu in breast cancer in this population, compared with findings in other populations.Both biomarkers were positively correlated with each other. Furthermore, all the cases that co-expressed both HER-2/neu and p53 showed the most unfavorable biopathological profile. CONCLUSION: P53 and HER-2/neu over-expression play an important role in pathogenesis of breast carcinoma. The findings indicate that in regions exposed to high levels of depleted uranium, although p53 and HER-2/neu overexpression are both high, correlation of their expression with age, grade, tumor size, recurrence and lymph node involvement is similar to studies that have been conducted on populations not exposed to depleted uranium. HER-2/neu expression in breast cancer was higher in this population, compared with results on non-exposed populations.
