Abstract

Background: The parasite Ascaris lumbricoides modifies the host's immunological response. This study aims to determine the interleukin-10 (IL-10) and interleukin-22 (IL-22) serum levels in specific patients and their associations with the severity of infection. The present study also requires to study a few blood markers in Ascaris lumbricoides-infected patients.Patients were referred to the laboratory for testing from Al Sader General Hospital in AL-Najaf Province, Al Furat General Hospital, AL-Zahra maternity and pediatrics, and AL-Hakeem Hospital. Samples Collected 32 samples were taken from patients and 20 healthy individuals who attended clinics at the AL-Sadder Teaching Hospital and AL-Hakeem Hospital in the province of AL-Najaf between July 2021 and June 2022. Male and female patients between the ages of 10 and 50 had their stool samples taken and put in clean, wide-mouth specimen bottles. The same patients' veins were also punctured to get five milliliters of blood, four milliliters of which were then deposited in specimen tubes. After 30 minutes at room temperature, stool samples were examined using the Formol-ether concentration and Kato-Katz thick smear procedures. According to the findings, ascariasis patients had significantly higher serum levels of interlukin-10 and interlukin-22 than the healthy group (P0.05). The findings showed that the iron level concentration in ascariasis patients decreased significantly (P0.05), and that the Hb, PCV, and RBC count also decreased significantly (P0.05).

Assessment of women's knowledge regarding perineal care after episiotomy

Abstract

Background: Today, episiotomy is generally reserved for cases where it is medically necessary or when a tear occurs naturally during delivery. Whether a woman has an episiotomy or experiences a tear during delivery, proper perineal care after childbirth is important for promoting healing and preventing complications . Methodology: Cross-sectional descriptive study design was conducted to identify knowledge of women concerning perineal care after episiotomy and to find out the relationship between women's knowledge and their demographic data (age, education level, residence, birth number, labor type, children number, educational level), the period of the study is from December, 1st, 2021 to May, 23rd, 2022. A non-probability sample (Purposive sample) of hundred (100) women who attended to teaching hospitals at Al-Najaf Al-Ashraf Health Directorate. Data collection was performed through the construction of a questionnaire using an Arabic questionnaire by interview technique with each woman. It consists of two parts: Part I Included Socio-demographic characteristics; Part II Includes (20) items concerned with perineal care after episiotomy. A panel of experts conducts study instrument validity with years of experience in the nursing field. Results: Most women have low knowledge of the care of the perineal body after episiotomy; there is a significant between the knowledge of women, the level of education, and the type of work. Conclusion: Most women have low knowledge regarding perineal care after episiotomy. Recommendation: For episiotomy care and placing them in hospital delivery rooms, emphasizing audio-visual mass media to publish information on the care of perineal after episiotomy. Hospital and health centre healthcare providers should educate women on the care of the perineal after the episiotomies during a prenatal visit.

The Role of Interleukin-6, and IL-12+ p40 in the Development of Ischemic Heart Disease

Abstract

Background: Because low-grade inflammation may play a role in the pathogenesis of coronary heart disease, and pro-inflammatory cytokines govern inflammatory cascades leading to atherosclerosis, and then to ischemic heart disease. Objectives: To evaluate the role of two cytokines IL-6 and IL-12+p40 in the etiology of IHD, as well as to observe the relationship of IL-6 , IL-12+p40 and two chronic diseases diabetes mellitus and hypertension in IHD patients. Methodology: A total of 70 (55 males and 15 females) patients with ischemic heart disease were included in the study. They were admitted to the Coronary Care Unit (CCU) at Ibn Al-Betar Hospital (Baghdad) for a surgical operation during the period July 2022 - April 2023. In addition to the patients, a total of 20 apparently healthy individuals (control group). Serum levels of IL-6 and IL-12+p40 were quantitatively determined in patients and control subjects by means of indirect sandwich Enzyme Linked Immuno Sorbant Assay (ELISA) test. Results: Serum levels of IL-6 for total patients are (404.8 pg/ml), diabetic (480.3 pg/ml), non-diabetic (250.8 pg/ml), hypertensive (451.6 pg/ml) and non-hypertensive (322.2 pg/ml), patients showed a significant increased serum level of IL-6 as compared to controls (155.7 pg/ml). The diabetic patients also showed a significantly higher level than non-diabetic patients (480.3 vs. 250.8 pg/ml), as well as, hypertensive patients (451.6 pg/ml) versus non-hypertensive patients (322.2 pg/ml). Serum level of IL-12+p40 for total patients (410.2 pg/ml), diabetic (357.7 pg/ml), non-diabetic (458.1 pg/ml), hypertensive (270 pg/ml) and non-hypertensive (420.5 pg/ml) patients showed a significant decreased level of IL-12+p40 as compared to controls (649.3 pg/ml). In contrast, the non-diabetic patients showed a significantly higher level than diabetic patients (458.1 vs. 357.7 pg/ml), as well as, non-hypertensive patients (420.5 pg/ml) versus hypertensive patients (270.0 pg/ml). Conclusion: proinflammatory cytokine cascades play crucial roles in the onset and progression of IHD.

الخالصة: خلفية البحث: متالزمة الجهاز التنفسي الحادة الوخيمة فايروس كورونا2- هو ثالث فايروس كورو نا قاتل ظهر في العقدين الماضيين, بعد فايروس كورونا المتالزمة التنفسية الحادة الوخيمة 1- ومتالزمة الشرق األوسط التنفسية. االهداف: للتحقيق في االرتباط بين عدوى كوفيد19- والبيانات الوصفية للمريض مثل الجنس والعمر ومراقبة االرتباط بين ferritin serum, CRP, Dimer-D, -6IL, في مرضى كوفيد.19- منهجية البحث: أجريت الدراسة خالل الفترة من 1 أكتوبر 2021 الى 1 مارس ,2022 تم جمع 110 عينة من مرضى كوفيد,19- الذين حضروا إلى مستشفى األمل التخصصي لألمراض االنتقالية في األسبوع األول من اإلصابة. تتراوح أعمارهم بين -15 90 سنة ومقارنتهم ب 50 فردا صحيا كمجموعة سيطرة. تم تشخيص كل حالة باستخدام تفاعل البوليميراز المتسلسل العكسي. تم جمع 7 مل من عينات الدم الوريدي من مرضى كوفيد19 لالختبارات المصلية والمناعية لقياس مستويات CRP, serum .IL--6 ,D-Dimer ,ferritin النتائج: في الدراسة الحالية, أنتشار شدة اإلصابة بكوفيد19- حسب العمر ازدادت لدى كبار السن واعلى إصابة في الفئة العمرية )-71 90( بنسبة %100 تليها الفئة العمرية )-51 70( بنسبة 87.2 % ثم سجل معدل أنتشار منخفض في الفئة العمرية )50-36( بنسبة 37.5 % وانخفاض معدل االنتشار 28.6 % في الفئة العمرية )-15 35(. من أجمالي المرضى, كان 79 )71.8 %( منهم من الذكور و 31 )28.1 %( من األناس. يظهر متوسط مستوى المصل للفرتين فروقا معنوية في مصل مريض كوفيد- 19 مقارنة بمجموعات السيطرة )392.4 مقابل 131.6 مايكرو غرام/لتر(, كذلك أظهر متوسط مستوى المصل ل CRP فرقا معنويا عاليا في مصل مرضى كوفيد19- بالمقارنة مع مجموعات السيطرة )59.26 مقابل 7.61 ملغرام/لتر(, ايضا متوسط مستوى المصل ل dimer-D اظهر فرقا معنويا عاليا في مصل مرضى كوفيد 19- بالمقارنة مع مجموعات السيطرة )1104 مقابل 220 نانو غرام/مل(. أظهر متوسط مستوى المصل ل-6IL في مرضى كوفيد19- فرقا معنويا عاليا بالمقارنة مع مجموعات السيطرة )130 مقابل 9.2 بيكوغرام/مل(, وكذلك أظهر فرقا معنويا عاليا في الحاالت الشديدة والحاالت الشديدة المصاحبة للوفاة عند مقارنتها بمجموعات السيطرة, باإلضافة الى ذلك أظهر مرضى كوفيد19- ارتباطا ايجابيا مهما ل -6IL مع .r قيم بواسطة CRP , D-dimer, ferritin االستنتاج: أنتشار اإلصابات الشديدة بمرض كوفيد19- حسب العمر تزداد عند كبار العمر من المرضى وأظهر الذكور نسب إصابة عالية اكثر من اإلناث. ارتفاع مستوى المصل ل ferritin and ,dimer-D ,CRP -6,IL مرتبط مع النتائج السيئة في مرضى كوفيد.19- التوصيات: يحتاج هذا النوع من الدراسة الى مزيد من التعاون بين المؤلفين, األطباء المتخصصين والمختبرات الطبية ألن هذه الدراسة مفيدة جدا للمرضى واألطباء لتقييم الدور الدقيق ألدوية كوفيد19- التي يمكن أن تتحكم في عاصفة السايتوكينات ومستوى CRP, ferritin serum, Dimer-D, -6IL, مما يؤدي الى إنقاذ حياة المرضى.

Abstract

Severe acute respiratory syndrome coronavirus-2 is the third fatal coronavirus that has emerged in the past two decades, following severe acute respiratory syndrome coronavirus-1 and the Middle East respiratory syndrome coronavirus . the prevalence of severe COVID-19 infection according to age are increased in older aged patients, the highest one appeared at the (71-90) age group in 100% followed by (51- 70) age group 87.2%, then the low prevalence frequency was recorded at (36-50 ) age group in (37.5%), and the low prevalence 28.6 % at the age group (15-35). From total patients, 79(71.8%) of them were males and 31(28.1%) were females. The mean serum level of ferritin show a significant differences in sera of patient with COVID-19 as compared with control groups (392.4 vs. 131.6 µg/l), as well as, the mean serum level of CRP show a highly significant difference in sera of patient with COVID-19 as compared with control groups (59.26 vs. 7.61 mg/L), also the mean serum level of D-dimer show a highly significant difference in sera of patient with COVID-19 as compared with control groups (1104 vs. 220 ng/ml). The mean serum level of IL-6 show a highly significant difference in sera of patient with COVID-19 as compared with control groups (130 vs. 9.2 pg/ml), also it show a highly significant differences in severe and severe with death cases when compared with control groups, in addition, COVID-19 patients show a significant positive correlation of IL-6 with CRP, Ferritin, and D-dimer by their r values.

Quality of Life for Residents in Nursing Home at Al-Najaf City

Abstract

Quality of life is an ideal health assessment that covers physical, psychological, social, and environmental life domains. It refers to the subjective evaluation of individual overall life satisfaction and wellbeing. In addition to the elderly have special requirements as their age increases. Hence, it is necessary to evaluate their quality of life from multiple dimensions. Furthermore, The world is aging today, there are about 600 million people aged 60 and over worldwide; this total will double by 2025 and will reach virtually two billion by 2050. The majority of older people will be living in developing countries that are often the least prepared to confront the challenges of rapidly aging societies.The study aims are to determine the quality of life for elderly residents in a nursing home at Al-Najaf city and to identify the relationship between quality of life for elderly residents and their socio-demographic characteristics as well as determine health services according to the opinions of the residents.Methods:A descriptive cross sectional, non-probability sampling (Convenience Sampling) that consist of (29) residents in a nursing home at Al-Najaf city. After determining the validity and reliability of the study instrument, data is collected through the use of the questionnaire and an interview approach. The validity of the questionnaire is assessed by 11 experts from various specialties, and the reliability is evaluated by assessing the study instrument using the Alpha Cronbach's approach. The descriptive and inferential statistical analysis methodologiesare used to analyze the data.Results: The results of the study assert that the socio-demographic characteristic among the elderly in a nursing home there does not significantly effect on the quality of life except age, gender, and smoking. Also, this study confirms that the correlation between psychological health and physical health which is a highly significant positive correlation. Conclusions: The study recommends improvement health services provided in the nursing homesuch as give nutritional supplements and visit specialists of Ear Nose doctors, Ophthalmologists, Rheumatologists, and Dentists to see residents regularly to improve the level of quality of lifeof them. Provide health education programs about physical activity as well as, provide equipment for practicing this activityand provide enough knowledge for the staff about psychological support.Attention to environmental healthand Promote resident dignity and respect.In addition, adding a general clinic for the purpose to give oxygen or for wrapping up dressing for emergency cases and Monthly laboratory examinations for those with chronic diseases as a preventive measure.
