Imaging plays integral part in staging of laryngeal cancer with computed tomography (CT) being among the most used modalities.With advance in CT manufacturing and widespread use of multidetector CT (MDCT), more accurate and dependable clinical application would be expected. Objective: To evaluate the role of MDCT in the staging of laryngeal cancer and to compare the efficacy of MDCT in assessing laryngeal cancer with respect to clinical and histopathological TNMstaging. Patients and methods: Twenty-four patients with endoscopically proven laryngeal carcinoma underwent MDCT examination during the period between February 2013 and October 2013at CT unit of Middle Euphrates Neurocenter in Al-Sadder Medical city, Al-Najaf city. Clinical and histopathological records of all patients were reviewed and compared with MDCT findings. Results: Twenty-four patients were included, with male to female ratio of 3:1 (18 males, 6 females) with mean age of 62.58±12.98 years. Fifty percent (12) of cases were at TNM stage 3, while stage 2 and stage 4 constituted 25% (6 cases) for each equally. There was statistically significant relation between stage of the tumor and volume. Male gender had statistically significant association with higher TNM stage while clinical presentation had not. Conclusion: MDCT has a crucial role in estimation of the TNM stage of laryngeal carcinoma.The TNM stage of the laryngeal carcinoma tends to be upgraded when following MDCT findings are present:heterogenous density of the mass, presence of cartilage destruction, presence of anterior commissure involvement and larger volume of the mass. However, presence of extra laryngeal extension on MDCT may have no significant relation to a higher TNM stage. Keywords Laryngeal carcinoma; Multiple detector computed tomography; Density of the mass, Cartilage invasion Introduction Laryngeal carcinoma is the eleventh-most common form of cancer among men worldwide and is the second-most common malignancy of the head and neck [1]. The assessment of the laryngeal tumor is based upon inspection and palpation, when possible, and by both indirect mirror examination and direct endoscopy [2,3]. Examination under anesthesia should be performed to best assess the extent of the tumor, to look for synchronous primary tumors, and to take biopsies [2]. Imaging studies, including computed tomography (CT), magnetic resonance imaging (MRI), and positron emission tomography (PET), may augment the physical exam and evaluation, particularly for assessing the degree of local invasion, involvement of regional lymph nodes, and the presence of distant metastases and second primary malignancies [4]. Ideally, imaging should take place prior to biopsy, which may distort anatomy and create a false positive finding on PET scanning [4,5]. Multiple detector computed tomography or (MDCT) scanning is a rapid, painless diagnostic procedure that combines the use of computers and X-rays. Multiple images are acquired in a sequence by a rotating x-ray tube. The patient is lie still on a table for approximately 5 to 15 min, depending on the area of concern being covered. Images are acquired by the detectors that pick up the X-ray that passes through patient body. The images are reconstructed for the assessment. For Head and Neck cancers, it is particularly useful in upstaging cancers that have deeper local invasion or infiltration into adjacent structures that is difficult to detect on physical examination [6]. Patients and methods: A prospective study of (125) consecutive tracheostomies that were performed during one year period from May 2006 through May 2007 in Medical City, Baghdad.Results: Ninety three patients (74.4%) were males. Thirty seven patients (29.6%) were in their third decade. Complications developed in fifty three patients (42.4%); wound infection was the commonest in twenty three patients (43.4%), followed by subcutaneous emphysema in fourteen patients (26.4%) and haemorrhage in ten patients (18.8%). One tracheostomy related death was encountered (0.8% of the total no. of patients). The complication rate was significantly higher in urgent tracheostomies (50.6%) than elective procedures (26.1%) and in those performed with local anaesthesia (48.3%) compared to (25%) under general anaesthesia.Conclusions: Most of early complications were due to the urgency of the procedure and inadequate postoperative nursing care. Wound infection was the commonest complications. تعتبر عملية تفويه الرغامي من العمليات الأمنة , ولكنها تحمل كأي عملية جراحية أخرى خطورة حصول المظاعفات.الهدف من الدراسة: تقييم المظاعفات المبكرة الحاصلة بعد عمليات تفويه الرغامي التي أجريت في مستشفيات مجمع مدينة الطب في بغداد .المرضى والطرق: دراسة منظورة ل (125) عملية تفويه رغامي متتابعة أجريت خلال فترة عام واحد من أيار 2006 وحتى أيار 2007 في مستشفيات مجمع مدينة الطب في بغدادالنتائج:33 مريضا(74,4%) كانوا ذكورا. 37 مريضا( 29,6%) من مرضى هذه الدراسة كانوا في العقد الثالث من العمر. حصلت المضاعفات عند 53 مريضا(42,4%).ألتهاب الجرح الجرثومي كانت الأكثر شيوعا وحدثت في 23 مريضا 43,4%),تبعها النفاخ تحت الجلد في 14 مريضا(26,2%) والنزف في عشرة مرضى(18,8%).سجلت حالة وفاة واحدة متعلقة بعملية تفويه الرغامي(0,8%).معدل المظاعفات كان مرتفعا بصورة ملحوظة في حالة العمليات الطارئة(50,6%) عن ماهو عليه في العمليات الباردة(26,1%) , وفي العمليات التي أجريت تحت التخدير الموضعي (48,3%) مقارنة بتلك المجراة تحت التخدير العام (25%).الأستنتاجات: أغلب المضاعفات المبكرة لعملية تفويه الرغامي كانت عائدة الى الطبيعة الطارئة للعملية والعناية التمريضية غير المناسبة بعد العملية. ألتهابات الجروح الجرثومية كانت هي الأكثر شيوعا بين هذه المظاعفات Keywords Tracheostomy --- urgent --- early complications IASJ is provided by Ministry of Higher Education & Scientific Research of Iraq ©2020 SemperTool

There is an old famous statement (when you think about tracheastomy, it is time to do it). This statement is far away from reality with the emergence of new less invasive methods of securing airway.Aim: to evaluate the indications of tracheastomy in Najaf.Patients and methods: a cross-sectional analytic study of 87 tracheastomy operations had been done in the period between January 2008 to February 2011 in Al-Sadar teaching hospital in Najaf.Results : The mean age of our 87 patient was (52.4). Most of our patients were males 59(67.8%). The main indication for tracheastomy was upper airway obstruction 59 patients (67.8%), While the least indication was subglottic stenosis 2,2%. Among upper airway obstruction causes, carcinomas of the larynx and hypopharynx were the main indications for tracheastomy 67.7%. From 20 patients reqired tracheastomy for assisted ventilation 65% had head injury. In those six patients whom the main indication of tracheastomy was for bronchial toilet, 83.3% had head injuries (5patients).Conclusions : our indications for tracheastomy in Najaf is quite different from those over all the world. While still relieving of upper airway obstruction is the most common indication in our study, replacement of prolong intubation represent the first indication in developed countries. تعتبر عملية تفويه الرغامي من الطرق الجراحية القديمة والتي لاتزال تستخدم بشكل واسع في الوقت الحاضر. ان المقولة القديمة الشهيرة(عندما تفكر بتفويه الرغامي عندها عليك أجراء العملية),هذه المقولة بعيدة كل البعد عن الواقع مع بروز الطرق الجديدة الأقل تداخلا لضمان المجاري التنفسية.الهدف من الدراسة تقييم مؤشرات اجراء عملية تفويه الرغامي في مدينة النجف الأشرف.تمت الدراسة بأثر رجعي شملت 87 عملية تفويه رغامي أجريت في الفترة الزمنية بين كانون الثاني 2008 وشباط 2011 في مستشفى الصدر التعليمي في النجف.النتائج :معدل اعمار المرضى في هذه الدراسة (52,4). معظم المرضى كانوا من الذكور(59 مريضا). المؤشر الأساسي لأجراء عملية تفويه الرغامي كان انسداد المجاري التنفسية العليا67,8 % واقل المؤشرات هو تضيق الجزء السفلي من الحنجرة(2,2%).من بين أسباب انسداد المجاري التنفسية العليا ,شكل سرطان الحنجرة وأسفل البلعوم السببالأساسي(67,7%).من بين 20 مريضا أحتاجوا أجراء عملية تفويه الرغامي لغرض مساعدة التنفس, 65% منهم كان لديهم أصابات في الرأس.6 من المرضى أجريت لهم عملية تفويه الرغامي لغرض تنظيف المجاري التنفسية ,83,3% كان لديهم أصابات بالرأس.الأستنتاجا ت: مؤشرات أجراء عملية تفويه الرغامي في النجف كانت مختلفة جدا عما هي عليه في الدول المتقدمة.ففي الوقت الذي ظل فيه تفريج أنسدلد المجاري التنفسية العليا السبب الأكثر شيوعا في النجف فأن أستبدال أنابيب التخدير الموضوعة لمدة طويلة أصبح المؤشر الأول لأجراء العملية في الدول المتقدمة. The pregnant woman should take special consideration due to changes that affect both the mother and her fetus and the relation between these changes and ENT problems. الخلاصة: يعد اختصاص طب وجراحة الأنف والأذن والحنجرة من الاختصاصات التي تتعامل مع عدد كبير من المرضى في العيادة الخارجية ومن كافة الأعمار ومن كلا الجنسين.وتعتبر المرأة الحامل ذات طبيعة خاصة يجب أخذها بالحسبان من قبل طبيب الأنف والأذن والحنجرة و ذلك للتغيرات التي تطرأ على المرأة وجنينها وارتباط بعض هذه التغيرات بأمراض الأنف والأذن والحنجرةالهدف من الدراسة: تقييم أمراض الأنف والأذن والحنجرة التي تحدث خلال فترة الحمل وبيان ارتباط بعضها بالتغييرات الفسلجية التي ترافق الحمل وتؤدي إلى أن تصبح هذه الأمراض أكثر شيوعا في فترة الحمل.المرضى والطرق: دراسة الحالات والشواهد ل155 امرأة تم تقسيمهن إلى مجموعتين, الأولى تألفت من 69 امرأة حامل ومثلت هذه المجموعة مجموعة الاختبار بينما تألفت مجموعة السيطرة من 85 امرأة من غير الحوامل وجميعهن قد تم أجراء الفحص السريري عليهن سواء في العيادة الاستشارية لإمراض الأنف والأذن والحنجرة في مستشفى الصدر التعليمي في النجف أو العيادة الخاصة للباحث للفترة من أيار 2009 ولغاية شباط 2011.تم استخدام برنامج SPSS لأجراء التحليل الإحصائي للنتائج المستخلصة.النتائج:معدل أعمار النساء الحوامل في هذه الدراسة كان 27,49 بينما كان معدل الأعمار للنساء في مجموعة السيطرة (غير الحوامل) 26,69 . أكثر أمراض الأنف والأذن والحنجرة شيوعا في فترة الحمل كان التهاب الأذن الخارجية (30,3%) تلاه التهاب الأغشية المخاطية التحسسي (20.7%) وأقلها شيوعا التهاب الغدة اللعابية النكفية والصمم الحسي العصبي وتمزق طبلة الأذن نتيجة شدة خارجية.حدث أكثر حالات التهاب الأذن الخارجية في الأشهر الثلاثة الأخيرة من الحمل.الاستنتاجات: أمراض الأنف والأذن والحنجرة تعتبر شائعة أثناء الحمل ومعظمها تختفي أو تتحسن بعد الولادة لذا يكون العلاج التحفظي مفضلا على غيره من الأساليب العلاجية الأخرى لتقليل الخطورة على الأم والجنين.
