Pulmonary tuberculosis (PTB) is still a global problem and serious epidemic disease and leads to a high death rate especially in developing countries, partially due to problematic ecological situation. The aim of this study to verifying the numbers and percentages of individuals infected with PTB in Iraqi Kurdistan Governorates 2019. A statistical–retrospective study was carried out to investigate the numbers and percentages of individuals infected with PTB in three provinces in Iraqi Kurdistan; Erbil, Sulaymaniyah and Duhok from the 1st January to 31st December 2019. All essential details of annual data for PTB patients were obtained from the National Center of PTB in Baghdad Governorate, Iraq. A total of 639 cases were recorded in 2019; 295 male (46.1%) and 344 female (53.9%). Erbil Governorate had the highest number of infections with 302 cases (47.2%) followed by Sulaymaniyah and Duhok with 214 (33.5%) and 123 (19.3%) respectively, the most cases were recorded in 2nd quarter with 170 cases (26.6%). The highest frequency of PTB was recorded in age groups 65–80 with 123 cases (19.2%) while the lowest was 5–14 with 20 cases (3.1%). Pulmonary TB remains a threat in Iraqi Kurdistan and about half of cases have been recorded in Erbil Governorate. The highest frequency of PTB was in females than males and the older age groups were the most infected. Therefore, we need more precautionary measures to control this disease.

مختصر البحث

Pulmonary Tuberculosis (PTB) is an airborne bacterial infection that can be deadly if not treated. PTB is most common in developing countries, ecologically problematic, where it kills about 1 million people each year. Pulmonary tuberculosis is a dreaded disease that can have a devastating impact on its victims. Tuberculosis is a serious respiratory infection caused by the bacterium Mycobacterium tuberculosis. It commonly affects the lungs but can also affect other parts of the body, such as the kidney, spine and brain. This review article discusses the PTB risks and challenges word wide. The global burden of TB remains high, with approximately one-third of the world's population being infected with the bacterium. Overall, more needs to be done to improve the management of TB worldwide in order to ensure that no person is left to suffer from the devastating effects of this disease.

مختصر البحث

Urinary tract infections (UTIs) are common in both men and women. They are caused by bacteria that get into the urinary tract through the urethra (the tube going from the penis to the bladder) or, in rare cases, the ureters (tubes that carry urine from the kidneys to the bladder). Risk factors for developing a UTI include having a catheter inserted to drain the bladder or a surgical wound on the urinary system. Pseudomonas luteola (P.luteola) is a Gram-negative, rod-shaped bacterium considered ecologically problematic and can cause pneumonia and urinary tract infections in humans. It is a common cause of hospital-acquired pneumonia and accounts for around 20% of all such cases. P. luteola causes fever, tachycardia, and other signs of infection, but some of these symptoms may be present for many weeks before the symptoms of infection are clear. According to our knowledge, this is the first detection in January 2023, of P. luteola in Iraq, Babylon City from a 51-year-old woman with chronic urinary tract infections and kidney failure (dialysis).

Urine infection, also known as a urinary tract infection (UTI), is an inflammation of the urinary system, while it is most common in women, UTI can occur in both men and women. A total of 719 urine samples were collected from women with UTI admitted to a general hospital in Babylon City, Iraq, between March 2022 and February 2023. Standard laboratory methods were used to culture urine and to diagnose all bacterial isolates that were grown on laboratory media. The Kirby-Bauer disc diffusion method was used to test antibiotic susceptibility. Out of a total of 719 urine samples, 646 urine samples with positive bacterial growth and 73 urine samples without bacterial growth. E.coli was the most predominant bacteria with 309 isolates, K.pneumoniae 112, S.saprophyticus 62, E.faecalis 48, P. aeruginosa 31, S.aureus 20, S.haemolyticus 19, St.agalatiae 16 and C. freundii 8 isolates. E.coli, K.pneumoniae and P. aeruginosa were highly resistance to amoxicillin 25 μg with percentages 81%, 82% and 90% respectively; S.saprophyticus had good resistance to penicillin 6 μg and ampicillin 10 μg with percentages 74%. The most effective antibiotic was imipenem 10μg, which inhibited all bacterial growth.

This research involved synthesizing two compounds as derivatives of 2-naphthol by the classical method. The azo coupling compound was prepared via diazonium salt of 4-amino acetophenone followed by 2-naphthole. The azo-Schiff molecule was synthesized through condensation of an azo compound with 5-amino salicylic acid in an acidic medium. Using spectrum approaches, the new compounds are characterized (13C-NMR, 1H-NMR, FT-IR). The acid-base characteristics were investigated at various pH levels (0.67-12), and the ionization and protonation constants were determined. Spectroscopic methods were used to identify new copper (II), nickel (II), and cobalt (II) complexes of the azo-Schiff base ligand. All the compounds were evaluated for antibacterial activity against bacteria such as Staphylococcus aureus (Gram-positive bacteria) and Escherichia coli (Gram-negative bacteria). It was found that the coordination complexes of Cu and Co had an antibacterial effect on both concentrations against Staphylococcus aureus.

Since Trichomonas vaginalis (T. vaginalis) only manifests as a trophozoite stage during sex, it is typically transmitted during sex as a trophozoite. The purpose of this study was to determine the concentrations of copper, iron, and Eosinophil cationic protein (ECP) in vaginitis patients. Between July and November 2021, 355 suspected patients who visited the AL-Zahra maternity and pediatrics laboratory in the AL-Najaf region (Iraq) and 30 healthy respondents of the same age participated in the study. According to the manufacturer's instructions, the ELISA technique was used to measure the levels of three biomarkers in serum (copper, iron, and ECP). On the other hand, the colorimetric approach was used to measure the copper concentration. The investigation showed that in the blood sample of T. vaginalis-infected patients, the amounts of copper and iron considerably decreased (P > 0.05). In contrast, the serum levels of ECP considerably increased (P > 0.05) when compared to the control group. As a result of the T. vaginalis infection, copper levels in the blood are dramatically impacted, according to the current study's findings. The current study results suggested that the T. vaginalis infection alters serum iron and ECP concentrations, significantly affecting copper levels in the blood.

This study aimed to determine the prevalence of Giardia lamblia infection in Al-Najaf province, Iraq, based on various risk variables for sex years. From October 2015 to March 2021, samples were collected from suspected patients in the AL-Hakeem hospital and AL-Zahra maternity and paediatrics clinics in AL-Najaf province; stool samples were collected into clean, wide-mouth specimen bottles from suspected patients, and freshly voided stool specimens were processed and examined microscopically for intestinal paralysis. The current investigation found that 44 out of 44 (9.77%) patients were infected after detecting cyst and trophozoite of G. lamblia. The infection rate is proportional to the size of the family. Children from tiny families had the lowest infection rate (18.3%/15/82) and 18.3% (15/82). (4 persons). The infection rate gradually increased as family size grew, reaching 83.3 percent (5/6) in children from big households (12 persons). Based on the chi-square test, it was discovered that family size is non-significantly (P > 0.05) connected with pinworm infection.

Nearly half of the world's population is infected with Helicobacter pylori (H.pylori). A duodenal ulcer or stomach cancer can be caused by the infection by this bacterium. The aim of this work is to assess the levels of CD14 and CD163 in H.Pylori-positive patients infected with duodenal ulcer (DU) and gastric cancer (GC) and determine the prevalence of Helicobacter Pylori-oncogenic protein cytotoxin-associated gene A strains (H.pylori-CagA). This study included 89 individuals distributed as follows: 20 healthy individuals as controls and 69 patients infected with H. Pylori have been divided as follows: 27 patients infected with H.pylori only (H.Pylori+), 22 H.pylori+DU and 20 H.pylori+GC. H. Pylori-oncogenic protein cytotoxin-associated gene A strains (H-pylori-CagA) were diagnosed based on a qualitative reverse-phase Enzyme Immunoassay Technique. CD163 and CD14 were measured in all individuals' serum using the Enzyme-Linked Immunoassay (ELISA) test. Out of 69 patients infected with H.pylori, there was one CagA strain in H.pylori+; two and five strains were recorded in H.pylori+DU and H.pylori+GC, respectively. CD14 and CD163 serum concentrations were significantly higher (P≤0.05) in H. pylori+, H. pylori+DU and H. pylori+GC than in controls. Conclusions: Patients with CagA strains infection are at risk of developing a duodenal ulcer and stomach cancer. CD14 and CD163 may serve as potential biomarkers for assessing the severity of H.pylori infection. The presence of elevated levels of CD14 and CD163 might be used as an early indicator of duodenal ulcer and gastric cancer in patients infected with H.pyloti.

Abstract: Citrobacter freundii is one of the most important pathogens that cause urinary tract infection in human because it harbors many types of antimicrobials resistance-associated genes. Therefore, this study aimed to evaluate the prevalence of antimicrobial resistance-associated genes citrobacter freundii isolated from patients with urinary tract infection in Al- Najaf governorate-Iraq. A total of 30 strains of citrobacter freundii isolated from urine samples of patients with urinary tract infection in Al-Najaf central hospital in Al-Najaf City, Iraq, during April to December 2018. Polymers chain reaction technique was used to detect the presence of antimicrobial resistance-associated genes. From the 30 strains of citrobacter freundii, 15 strains (50%) were positive for bla-tem gene, 16 strains (53.3%) were positive for bla-shv gene, 21 strains (70%) were positive for bla-ctxm gene, 3 strains (10%) were positive for bla-ctxm-1 gene, 6 strains (20%) were positive for bla-ctxm-2 gene. Interestingly, it should be noted that no any strain form the 30 we tested was carrying pse1, bla-ctxm-8, bla-ctxm-9 or bla-ctxm-25 genes. Our data showed that 19 strains (63.3%) were positive for cat1 gene, 9 strains (30%) were positive for cat2 gene and 4 strains (13.3%) were positive for cat3 gene. On the other hand, 17 strains (56.6%) were carrying qnrA gene, 11 strains (36.6%) were carrying qnrB gene and aac(6')- Ib-cr gene. Also, the results proved that there were 7 strains (23.3%) being positive for sul1 gene and 11 strains (36.6%) were positive for sul2 gene. We also found 22 strains (73.3%) being positive for strA-strB gene and 4 isolates (13.3%) were positive for aacC1 genes. No strain carrying aacC2, tetA and tetB genes was identified. Citrobacter freundii harbor large numbers of different antimicrobial resistance-associated genes that enable this pathogen to escape the effect of antibiotics which make this bacteria highly resistant causing urinary tract infection.

Introduction: Vaginal abscess in pregnant women caused by Staphylococcus aureus and Trichomonas vaginalis is one of the most important disease, this infection is still medically uncontrolled because of poor health awareness due to the absence of personal hygiene in some women. Aim: This study is aimed add evaluation of immune response in pregnant women infected with acute vaginal abscess caused by S.aureus and T.vaginalis due to the determination of four biomarkers level; Interleukin13 (IL-13), Prostaglandin E2 (PGE2), Transforming growth factor β1(TGF-β1) and Lactoferrin. Materials and Methods: This is a case control study was done in AL-Zahra hospital in AL-Najaf City, Iraq from January 2017 to November 2017. 120 pregnant women were included in this study, 30 women infected with S. aureus, 30 women infected with T. vaginalis, 30 women infected with both pathogens (inclusion criteria in this study) and 30 healthy pregnant women as control. All other cases were excluded from this study. Enzyme Linked Immunosorbent Assay (ELISA) technique was used in this study to estimate biomarkers level in serum of pregnant women infected with acute vaginal abscess. Results: The results of the current study proved that there was significant increase (P <0.05) in IL-3, PGE2 and TGF-β1 levels in serum of women infected with acute vaginal abscess caused by S.aureus, T.vaginalis and both pathogens as compared with control; while, there was significant decrease (P <0.05) in lactoferrin level in serum of pregnant women infected with T.vaginalis as compared with women infected with S.aureus and control. Conclusion: The synergistically pathogenic effect of S.aureus and T.vaginalis in pregnant women infected with acute vaginal abscess lead to activation of T-cell and overproduction of IL-3, PGE2 and TGF-β1 as a good signaling markers against infection. In contrast, this synergistic effect leads to reduction of lactoferrin level in the same women. That mean these women may be susceptible to acute anemia that may be lead to abortion.

Many strains of Klebsiella pneumoniae can produce several virulence factors, such as siderophores, capsules, fimbrial adhesins and antibiotic resistance enzymes, which are important in the adherence, colonization, invasion and development of the infection. Therefore the aim of this study was to investigate ten virulence gens in 32 K. pneumoniae isolated from different clinical infections: urine (13), burns (12), sputum (4) and blood (3). Phenotypic methods were used to detect capsule, hypermucoviscosity, ability to form biofilm, produce siderophores, primary and confirmatory test of extended spectrum beta lactamase (ESBL). Polymerase chain reaction (PCR) technique was used to detect ten virulence genes encoding attachments (fimH, ycfM), siderophores (kfu: iron uptake system, entB: enterobactin, irp-2: yersiniabactin), capsule synthesis or invasions (rmpA, uge, wabG) and beta lactamase enzymes (SHV, TEM). Antimicrobial susceptibility was tested by disk diffusion method. According to phenotypic methods, the results demonstrated that, the capsule and siderophores production and biofilm formation were observed in 100% of isolates, while hypermucoviscosity phenomenon was observed in 62.5%. The most common virulence genes were fimH-1, ycfM and entB (100%), uge and TEM (93.75%), wabG and SHV (87.5%). Kfu and rpmA genes were found at medium rates of 65.62 and 62.5%, respectively and at lower prevalence was gene Irp-2 (37.5%). A rate of 84.375% of isolates showed a multidrug resistance (MDR) pattern, 12.5% extensive drug resistance (XDR), 3.12% pandrug resistance (PDR) and the rate of extended-spectrum β- lactamases (ESBL) producing K. pneumoniae was 62.5%.The study concluded that fimH-1, ycfM and entB genes were commonly found in all isolates, they seem to be at the basis of classic pathogenicity of K. pneumoniae. The most common virulence genes were observed in isolates from burns and blood samples. K. pneumoniae became highly resistant to antibiotics especially to 3rd generation cephalosporins and there was positive relationship between presence of virulence genes and ability of the bacterium to antibiotics resistance.

Pyelonephritis is an inflammation caused by bacteria that primarily effects the interstitial area and the renal pelvis or, less often, the renal tubules. It is one of the most common renal diseases. The main objective of the study is to know and to compare the bacterial species that cause pyelonephritis in its three types, acute, chronic and emphysematous for patients in Iraq, specifically in the Al-Najaf Governorate. 663 (82.88%) samples gave a positive result for bacterial growth, and they belonged to patients with acute pyelonephritis 128 samples, chronic pyelonephritis patients had 288 samples and 247 samples for pyelonephritis emphysematous patients. The chronic pyelonephritis type had the most diverse bacterial species (11) bacterial species, the acute pyelonephritis (9) bacterial species, and the emphysematous pyelonephritis (6) species only. Escherichia coli bacteria was the most widespread of all types pyelonephritis, with a percentage of 34.41% (53/ 154) .For the acute type , percentage of 49.83% (152/305) was for the chronic type and a percentage of 56.07% (157/280) for the emphysematous type. 15 types of antibiotics were tested, and the antibiotics Imipenem and Amikacin were the best for most isolated bacterial species. In terms of gender, age and residence of the patients, females were the most susceptibility 341 (51.43%), and the age group (41-55) years is the most influential 211 (31.83%) and the percentage of residents of urban areas 379 (57.16%) gave the highest percentage of residents of rural areas. Infections of the Urinary tract are the most common infection affecting the human population of all ages. Escherichia coli is a bacterial agent with greatest recurrent cause of UTIs and cause the majority of infections, with other trace bacterial species accounting for a smaller proportion. These pathogens enter the urinary tract from the intestinal biotope, causing dysbacteriosis. Ninety one UTI samples were collected from patients at Sadr Medical City. The sample collection was done from May to August, 2022. By VITEK2 system all isolates were identified; all isolates were tested for sensitivity to 12 antibiotics. In this study, 75 samples (82.4 %) of the 91 samples (urine) were gram-negative bacterial isolates, 14 samples (15.38 %) were gram-positive bacterial isolates, and 2 samples (2.19%) showed no growth. The results revealed that (100%) of isolates have a high resistance rate to Cloxacillin, Ceftazidime, Penicillin, and Piperacillin, followed by Cefotaxime and Ceftriaxone (74.13%) isolates, Cefixime and Cefoxitin (68.96%) isolates, and Ciprofloxacin and Levofloxacin. In conclusions: Antibiotic resistance is still prevalent among pathogenic bacteria that cause urinary tract infections in pregnant women, which can cause dangerous complications to the fetus and the mother.

Abstract: Type 2 diabetes mellitus (T2DM) has become a significant global health care problem and its reported incidence is increasing at an alarming rate. Our study was conducted from December 2023 to April 2024 and the aim of the study is to a levels hemoglbin and vit d3 for men only in Najaf Governorate / Iraq withType 2 diabetic males and compared to healthy controls. The number of samples was 258 subjects (one hundred and fifty-two T2DM patients and ninety-six healthy controls). Ages Participants (20-60 years) showed that in general there is no significant difference, between the ages of patients and healthy people. When measuring the criteria for the studythe results showed that the hemoglobin level was within normal limits for both groups and did not constitute any significant difference at P-Value less than 0.5, for all ages. As for vitamin D3, there was a very slight increase in the control group, but it did not constitute any significant difference. Whatever the cause or result must be randomized controlled larger studies, which research the relation between diabetic regulation and vitamin D3 and hemoglobin levels.

Cancer is a disease in which some body cells grow uncontrollably and spread to other parts of the body, and its spread rate increases significantly in most countries, including Iraq. This study was conducted at the Islamic University/College of Medical Technology, where data was collected for 108 patients. Cancer from the Teaching Oncology Center in Najaf for the period from February 2023 to March 2024. The results revealed that there were 67 (62.04 %) female patients and 41 (45.37 %) male patients, and the age group (41-60) was the most prevalent, 45 (41.67%) patients. As for the type of cancer, breast cancer constituted the largest percentage, with 29 (26.8%) patients, followed by lung cancer, with 8 (7.4 %) patients. Prevalence of leukemia represented was (3.7%,) and it was equal among males and females, and most of them were under 20 years old (75%). One of the ways to reduce the spread of this deadly disease is to conduct a regular examination to detect any abnormal growth in the body, as early detection may accelerate recovery to a very large extent.

Abstract. Escherichia coli and Proteus mirabilis are two of the most common causes of upper or lowermurinary tract infection. The purpose of this study was to compare demographic characteristics and antimicrobial susceptibility of chronic pyelonephritis (CPN) between E. coli and P. mirabilis, and conducted by taking clinical samples urine from 288 patients with chronic pyelonephritis, including females and males, aged between 10-84 years, who attended Hospital Al-Sadr Teaching Urology Department in Najaf Governorate, Iraq from February 2023 to January 2024. The results showed that 17/288 (6%) were mix growth and 271/288 (94%) were single growths, which were focused on in this study (monomicrobe). The Gram-negative bacteria were 231/271 (85%) and the Gram-positive bacteria were 40/271 (15%), and E. coli bacteria constituted 119/231 (51.5%) and P. mirabilis, bacteria 27/231 (11.7%). The average ages were (52.92 ± 17.46) and (56.78 ± 18.03) for E. coli and P. mirabilis, bacteria, respectively. Significant differences were found at P-Value 0.5 in some age groups, in addition to both bacteria in males more prevalent with a significant difference. The presence of E. coli bacteria was more widespread in urban areas, unlike bacteria. P. mirabilis, which was more widespread in rural areas. Ten antibiotics were tested to compare them, and there were four antibiotics, namely Augmentin, Amikacin, Trimethoprim and Imipenem, which had the same effect and did not constitute any significant difference. As for the other types, they formed a significant difference.
